Fixed term/Secondment - 23 months.

This post offers an exciting opportunity to work with specialist nurse colleagues in the award winning drug redesign project team within primary care.

It will provide support to primary care in the management of opiate and alcohol dependence by supporting the transition of people from the specialist services into GP care, ensuring continuity of care and providing support at points of crisis within a primary care environment.

Key responsibilities include:

  • Holistic case management across Dundee City, working with a friendly, highly supportive, innovative, and forward thinking team focused on caring for people with opiate or alcohol problems.
  • Participation in the assessment, planning, implementation and evaluation of care for patients with complex drug and/or alcohol dependence with emotional and psychological needs.
  • High level liaison between Primary Care and DDARS, supporting people as required and working as part of a multidisciplinary team that includes partner agencies.
  • Proactive contribution to service delivery and development to support strategic objectives, with a commitment to quality governance and practice and excellent communication skills.
Qualifications

Must be registered on Sub part 1: RN1 or RNA or Sub part 2: RN2 UK NMC Register.

Hours of Work

36 hours per week.
