Discharge Facilitator

At present, University Hospitals of North Midlands is unable to offer visa sponsorship for Band 2 to Band 4 roles, as these positions do not meet the minimum salary and skill thresholds required under UK Visas and Immigration (UKVI) regulations.

Discharge Facilitator - Band 3 - Acute Medical Unit County. An exciting opportunity has arisen for enthusiastic, highly motivated individual to join our team on the Acute Medical Unit County Hospital - as a Discharge Facilitator.

Main duties of the job

You will assist the qualified nurses to plan and facilitate timely discharge processes for patients from point of admission to the ward environment to the discharge of patients to their own home or alternative care facilities. The post holder will have excellent communication skills and have the ability, drive and enthusiasm to work within a motivated team to provide high quality patient care. The post holder will ideally have experience as a Discharge Facilitator or relevant skills.

Person Specification Qualifications
  • NCQ level 2 in health & social care
  • NVQ level 3 in health & social care
Experience
  • Knowledge of community services/DST process/Social care etc
  • Good level of literacy and numeracy
