Meath.This role involves working closely with clients and community-based care teams to deliver high-quality, person-centred support in a home setting. It is ideally suited to someone who excels in leadership, care coordination, and client advocacy. Key Responsibilities Oversee and coordinate individualised care packages, ensuring safe, effective, and tailored support Provide clinical leadership, supervision, and guidance to community-based staff Develop, implement, and regularly review personalised care plans Liaise with families, multidisciplinary teams, and healthcare professionals to ensure integrated service delivery Conduct audits, compliance checks, and staff training to maintain regulatory and quality standards Support recruitment, onboarding, and supervision of care staff Assist with rostering and scheduling to ensure smooth operational delivery Essential Requirements BSc in General Nursing or Registered Intellectual Disability Nursing (experience in ABI is desirable) 2-5 years' experience in community or general nursing Active NMBI registration (essential) Full driver's licence and access to a private vehicle (travel required) Strong IT proficiency, including Microsoft Word and Excel Excellent organisational skills with a proactive and structured approach Strong communication and leadership abilities A commitment to continuous improvement and client-centred care Opportunity to lead and make a tangible impact in community disability services Supportive and collaborative working environment Competitive salaryOngoing professional development and training opportunities
