Childrens and young peoples Continuing Care,covering Buckinghamshire, Oxfordshire and Berkshire West (BOB) are looking forassessors with a range of clinical skills to support assessments of Childrenand young people with varying but complex health needs.

We are a team, keen to make a difference with plenty ofexperience and knowledge. There will be ample opportunity to develop new skillsworking alongside existing learning disability, mental health and physical health case managers leading theway in the application of Childrens Continuing Care Framework to supportequitable, good quality health care delivery and positive health outcomes.

Main duties of the job

If you have experience in complex physicalhealth and /or mental health and/or learning disabilities, you may be just theperson to join our thriving team. We are looking for individuals who are motivatedto use their clinical expertise to make a positive difference, deliveringassessments using the National Framework and seeking CYP and family basedsolutions often through Personal Health Budgets. As we cover a largegeographical area there will be opportunity to have a primary base in one ofour counties however there will also be an expectation that work is carried outacross BOB in answer to referral receipt and clinical need. Our currentassessors primarily work virtually however there is an expectation that face toface assessment and visits to home and school are part of each assessment.

Job responsibilities

Using clinical expertise in applying the Continuing HealthCare Framework for Children, the post holder will be responsible for Assessingand Reviewing individuals within or referred into CYPCC.

The post holder will work with wider teams to support thedelivery of individualised care delivery including Personal Health Budgets(PHBs).

Working within the remit of the National Framework (NF) for CYPCC,the post holder will provide a high quality service that supports the deliveryof an effective and competent service that consistently delivers anindividual-focused intervention with patients, their families andrepresentatives.

Take a lead on ensuring transitionprocesses are continually developed and improved to achieve seamless CYP keytransition milestones.

The post holder will be required to have a good knowledge of healthand social care practice and be able to demonstrate an understanding of CYPCCand be prepared to extend their skills, knowledge and competence in this field.

This post will require good communication skills withindividuals and their families or representatives and requires the ability towork in a multi-disciplinary way with colleagues.

Deliver against objectives, achieving quality outcomes,prioritising own workload and working to tight deadlines.

The post holder will be responsible for ensuring theirprofessional registration and mandatory training is up-to-date.

Experience
  • Extensive knowledge of National CYPCC Framework and a Personal Health Budget delivery.
  • Must understand the background to and aims of current healthcare policy and appreciate the implications of this on engagement.
  • Have an appreciation of wider Children's services delivery and shared responsibilities to meet need of CYP.
  • Have a minimum of two years experience working within Children's services.
  • Have experience working within Children's services in both acute and community settings.
  • Have working knowledge of safeguarding principles and processes
Communication Skills
  • Must be able to articulately navigate complex presentations and reflect these within any assessment clearly identifying points at which views differ.
  • To be able to maintain positive communication with families during highly emotive conversations.
  • To be able to communicate complexity and/or questions posed in a manner that can be understood well by non-medical personnel.
Qualifications
  • Hold a post graduate diploma qualification in a relevant health or care discipline or equivalent experience.
  • Member of relevant professional body.
