Crisis Practitioner Mental Health - North West Crisis

The organisation delivers acute hospital, primary, community and mental health care to a population of over 1.15 million (2.2 million including regional and national services).
An exciting opportunity has arisen to join the North West Crisis Team as a Crisis Practitioner. This is a full-time post of 37 hours per week, and it is a 7 day service with shifts of 9-5 or 12-8. It is essential you are an NMC registered Mental Health Nurse. This post is based within the North West area of Glasgow.
Responsibilities- Provide rapid, specialist mental health and risk assessment with a robust treatment plan for individuals in acute crisis.
- Offer treatment safely in a community setting as an alternative to hospital admission where appropriate.
- Minimise the length of stay in acute inpatient settings by supporting home passes or discharge when clinical risk can be managed in the community.
- Collaborate with inpatient staff to identify individuals whose care needs can be met in a community setting.
- Work across the North West area of Glasgow, based within 4 Community Mental Health Teams.
- Essential: NMC registered Mental Health Nurse.
- Effective verbal and written communication skills, with the ability to liaise with carers, inpatient services, Community Mental Health Teams, social work and third sector agencies.
