Community Wellbeing Link Worker (Social Prescribing)

A community support organization is seeking a Social Prescriber in the High Peak area. The role involves working with individuals to address health and wellbeing issues by linking them with community services. Successful candidates will ideally have experience in charity or healthcare settings. The position requires home visits, the co-production of wellbeing plans, and developing relationships to promote health and independence. Flexibility for evening and weekend meetings may be required.
