Community Wellbeing Link Worker

NHS is seeking a dedicated Social Prescribing Link Worker based in Newcastle upon Tyne to empower individuals to manage their health through community support. You will hold a caseload and work with a variety of agencies to improve wellbeing and reduce health inequalities.

The role requires experience in managing complex cases, excellent communication skills, and an understanding of social prescribing approaches. Starting salary is £28,116.80, negotiable based on experience.
