Community Reablement Assistant Level 2 - Enable Independence

Hampshire County Council is seeking a Community Reablement Assistant (Level 2) to support adults over 18 at home, helping maximise independence and reduce ongoing care needs. You will work with Occupational Therapists, Sensory Officers and health colleagues to enable safe, effective care in the community.

The role involves initial home visits, risk assessments, goals setting and mentoring of junior staff, with travel across Central Hampshire and occasional out-of-area work.
