Community OT Lead - Home First (Band 6)

Central London Community Healthcare NHS Trust in Brent is seeking a Band 6 Occupational Therapist to join the Home First Service, delivering rapid assessments and discharge planning to support independence at home.

You will manage a complex caseload in a 7-day service, work within a multidisciplinary team, and contribute to coordinated care with partners across health and social care, including supervision and training of junior staff.
