An exciting opportunity has come up to join our busy Neighbourhood Team. We are looking for an enthusiastic, hardworking, Band5 Community Nurse to work 37.5 hours per week or 30 hours per week. Community Nursing is an integral part of the NHS 5-year plan with an emphasis now being placed on admission prevention.
You will need to be passionate about community services and shaping a new way of delivering care.
We are an integrated team who work collaboratively and compassionately with therapists, local authority services, the urgent care response team and our colleagues from the Primary Care Network to provide patient centred, responsive care which meets the complex and varying needs of our housebound community.

- Registered Nurse (RN1)
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
- Accredited courses in palliative care, tissue viability, Diabetes
- Practice Assessor training
- Experience gained during Pre-Registration training
- 2 years post registration experience
- Community nursing experience
- Experience of management of caseloads
- Ability to challenge poor behaviour
- Role modelling good behaviour that supports teamworking
- Responsive to constructive feedback
- Ability to praise and be supportive to others
- Ability to communicate effectively and clearly in English in both written and verbal formats
- Clear and accurate documentation skills
- Ability to understand professional and ethical issues

- Variety of evidence based clinical skills
- Confidence to teach student nurses, trainee nursing associates, patients, carers and members of the multidisciplinary team

- Management of syringe drivers
