Community Mental Health Nurse - Band 6 post
Location: Doncaster Central Locality Adult Mental Health Team.
Salary: AFC Band 6
Hours: 37.5 hours
The Adult Community Mental Health Teams within Rotherham, Doncaster and South Humber NHS Foundation Trust have an exciting opportunity for a Band 6 community mental health nurse to join the Central AMH Locality Team.
The Central Locality Team provides support to individuals with complex and enduring mental health issues. The team offers a range of management strategies and interventions including various psychological therapies.
Applicants must demonstrate a flexible approach, evolving around the needs of service users, their families and significant others. The role involves working as a team member, with the responsibility of a caseload across traditional boundaries of Adult and the voluntary sector services. The service ethos is focused on recovery and social inclusion and therefore offers successful applicants an exciting and demanding opportunity to improve people's lives.
We are looking for an enthusiastic person with excellent team working skills, who is keen to develop their knowledge and skills in this area. You should be a competent and motivated practitioner with appropriate demonstrable experience. You will work as part of a multi-disciplinary team providing assessment and therapeutic intervention in the community.
Main duties of the jobTo deliver a health and social care service working as an integral member of the Community Mental Health Services providing a service for people experiencing / recovering from severe and enduring mental health problems.
The post holder through case management will lead and contribute to the provision of a comprehensive range of community services as a key worker for a defined caseload.
To be accountable for a clinical caseload delivering appropriate and effective evidence based care to meet the needs of the individual.
To work collaboratively with patients and their carers to facilitate and promote rehabilitation and recovery helping patients to achieve and maintain their optimum level of mental health functioning and quality of life in the community.

Person Specification Qualifications- Degree, Diploma or equivalent in R.M.N, and significant pre-qualification experience plus additional training/short courses
- Continuing post qualification Mental health training
- Experience of working with People with mental health problems
- Appropriate experience either pre or post qualification in the field of mental health
- Crisis working Management of complex needs
- Work in multi-disciplinary Setting
- Local Government experience
- Mental Health experience
- Care Programme Approach
- Care co-ordination.
- Experience of working with Older people with mental health problems
- Understanding of Equal Opportunities' Policy.
- Knowledge of the National Health Service and related organisations.
- Knowledge and understanding of current mental health issues
- Knowledge of the Mental Health Act and Mental Capacity Act and Care Act including other relevant legislation.
- Understanding of the Care Programme
- Approach and ability to fulfil a care co-ordinator role.
- Assessment and organisational skills.
- Ability to prioritise work.Range of intervention skills
- Evidence of co-ordinating /planning interagency approaches to mental health care.
- Ability to work as part of a team and also without direct supervision.
- Excellent communication skills in individual and group settings High standards of literacy and IT skills
- Experience in mental health work
- Experience in delivering psycho-social interventions
