An excitingopportunity has arisen to join our dedicated Frailty Team as a CommunityHealthcare Assistant.
This is arewarding and varied role where you will play a key part in supporting patientsto remain safe, healthy, and independent within their own homes and local carehomes. You will be confident working autonomously in the community while alsocontributing as part of a supportive multidisciplinary team.

Main duties of the jobKey Responsibilities:
- Takingand accurately recording patient observations
- Promotinghealth education and wellbeing initiatives
- Liaisingwith internal and external multi-disciplinary teams
- Providingadministrative support to the frailty team
