Community Health Navigator: Signposting & Care Access

HCRG Care Group in Chippenham is hiring a Community Health Navigator to be the first point of contact for patients, carers and professionals, guiding them to the right community health services. The role supports enquiries by phone, email and post, handles referrals, and maintains confidential records with compassion and professionalism.

The position offers either part-time or full-time hours (22.5-37.5 per week), a daytime service across 7 days, and on-site training with potential days from
