The ideal candidate will be able to demonstrate the relevant qualifications, clinical skills and professional registration required for this role, along with the confidence and sound judgement to make independent clinical decisions.

The clinic operates Monday to Friday 08:30 - 16:30 excluding bank holidays.
This post is offered up to 37.5 hours per week.
Main duties of the jobAs a Leg clinic nurse you will be providing an excellent standard of person centered care to adults with a lower leg wound in a clinic setting and occasionally their own home.
You will participate in the planning and efficient running of the leg ulcer clinic at Kirkley mill and may on occasion work from other clinic sites.
This role includes triage of incoming referrals, determining clinical priority, completing holistic patient assessment, specific wound assessments including recording dopplers, diagnosis and treatment plan, for patients referred to the service. Supporting patients to self care using health coaching.
You will be supported to complete the leg ulcer management course delivered in partnership with Accelerate CIC, and a health coaching qualification. You will be encouraged to continue your own professional development and contribute to service improvement. You will be part of the PCH support for students; those on an apprenticeship route and newly qualified nurses. Once experienced you will be a knowledgeable resource on leg ulceration to the wider PCH; other ECCH clinicians; primary care and other partners.

Job responsibilities- Contribute to, support and promote ECCH's, strategic direction, values and culture in relation to proactive services.
- Communicate effectively with patients, relatives and carers, respecting patient confidentiality with privacy and respect for diverse cultural backgrounds and requirements.
- Communicate complex, sensitive or contentious information to patients, relatives or carers, where persuasive, motivational, negotiating and empathetic skills are required.
- To act as a resource for community nursing colleagues and other health professionals regarding leg ulcer management across the Primary Care Networks.
- To participate in teaching and clinical supervision of support workers and other provider staff as required.
- To provide mentorship to pre and post registration students.
- To contribute to the clinical governance agenda through participation in clinical risk assessment and management.
- To critically evaluate research findings, national guidelines and support the implementation of changes in clinical practice as appropriate.
- To participate in clinical trials as required.
- To make a positive contribution to excellent communications within the organisation.
- To contribute to the delivery of the Lower Limb Pathway, providing management of lower leg ulceration in a clinical setting within agreed time frames.
- To create an environment conducive to effective working, respecting and supporting staff to deliver high quality clinical services.
- To work with other members of the primary care home to deliver pro-active services.
- Flexibility to work across bases within the organisation if required.
- To work with colleagues to contribute in developing care pathways for patients with a range of healthcare needs.

- Ensure own compliance with the Nursing and Midwifery Code of Conduct.
- Ensure a high standard of record keeping is achieved in line with ECCH and professional standards.
- To report any concerns via datix incident reporting system.
- To take responsibility for management of clinic resources.
- To contribute to the ongoing review of the wound care formulary and guidelines.

- Professional Registration with Nursing and Midwifery Council. (NMC) Registered Nurse Level (Adult) BSc in Nursing practice or equivalent
- Leg Ulcer Management Module at Degree level
- The ability to work unsupervised. Evidence of continuous professional development. The ability to demonstrate a comprehensive level of clinical knowledge and competence within the clinical sphere of practice. Evidence of experience in working to a case management approach.
- Mentoring. Experience of working in a community setting.

- Competent in application of compression bandaging as part of a treatment package to manage lower leg ulceration.
- Competence in Health Coaching, supporting patients to change health behaviours and beliefs.
- The ability to perform and interpret Doppler readings as part of a holistic assessment or be willing to learn.
