Assess Needs: Conduct thorough needs and risk assessments in line with the Care Act 2014 and Mental Capacity Act 2005.

Coordinate Support: Work collaboratively with social workers, occupational therapists, and other agencies to provide comprehensive care.
Connect Communities: Link individuals with local services and resources to promote self-directed support.
Respond to Inquiries: Address questions and concerns from clients and their families with empathy and professionalism.

To excel in this role, you should have:

A relevant qualification in social care or a related field.
Experience in adult social care or community support.
Knowledge of legislation affecting social care, including the Care Act 2014 and Mental Capacity Act 2005.
Strong assessment and analytical skills.
