Community Cardiac Rehabilitation Specialist Nurse (Development Post) NHS AfC: Band 6-7 Main area: Cardiac Rehabilitation. Grade: NHS AfC Band 6-7. Contract: Permanent. Hours: 30 hours per week (Mon Fri , may change for annual leave). Part time. Flexible working.

Duties and Responsibilities
- Be professionally accountable for maintaining standards and professional practice as set by Trust Policies, the NMC Code of Professional Standards of Practice and Behaviour, and relevant professional guidelines.
- Work autonomously to manage a caseload of complex cardiac patients in a variety of settings, achieving and maintaining regional and national targets throughout the patient's care episode.
- Become an independent prescriber and prescribe evidence based medicines within the appropriate scope of practice.
- Complete the service competency framework to a proficient standard before promotion to Band 7 is considered.
- Provide a planned and unplanned responsive service to maintain routine and anticipatory care for heart disease patients.
- Provide specialist nursing advice to support individualized care to patients and relatives/carers without supervision and give peer to peer support to the wider teams.
- Carry out a variety of physical observations using a range of equipment, record and report and raise exceptions as required.
- Ensure patient pathways are in place to increase referrals to cardiac rehabilitation, ICD/CRT device therapies, palliative care and to cardiology specialists.
- Provide mentorship and leadership support for staff developing their roles within the team.
- Collaborate with the service lead on ongoing development of policies and protocols to enhance service delivery.
- Maintain and update personal clinical expertise in accordance with the professional regulatory body.
- Monitor the quality of nursing care and act on audit information to achieve agreed standards.
- Implement specialist advice and demonstrate autonomous evidence based treatments to improve quality of care.
- Assess and evaluate inbound referrals, accepting or declining according to the service specification.
- Triage referrals based on a needs based approach.
- Clinically assess, plan, implement and evaluate individual care plans in patients' homes or community clinics.
- Manage individual patient caseload and support the wider team in safe discharge.
- Maintain electronic or written documentation in line with professional registration requirements and Trust standards.
- Lead clinical leadership initiatives and ensure high standard holistic, patient centred care.
- Engage service users and incorporate feedback from user groups, open days and support groups.
- Adhere to the Trust's framework for Clinical Governance.
- Identify clinical risk and act in accordance with the Trust's Risk Management policy.
- Provide venepuncture as needed, in line with Trust policies.
- Support patients and families managing coronary heart disease and heart failure, including palliative care referrals.
- Facilitate cost effective specialist service through monitoring, audit and quality initiatives.
- Provide visible and accessible professional nursing leadership.
- Work confidently within multidisciplinary teams and across partnerships.
- Participate in regular multidisciplinary team meetings, clinics and patient reviews.
- Maintain seamless service with key stakeholders to deliver highest quality care.
- Uphold the Trust's shared values and promote the corporate image of the Trust.
- Advocate for patients, challenge poor practice and support safety and safeguarding.
- Promote healthy lifestyle and facilitate positive change through motivational interviewing.

- Registered Adult Nurse on the NMC register.
- Post registration qualification in a relevant specialism or professional knowledge, supplemented by specialist experience and short courses to degree/masters level.
- Recognised Teaching/Mentorship module or equivalent experience.
- NMC registered Non Medical Prescriber (V300) or willingness to undertake.
- Working towards or holding a Masters qualification.
- Advanced communication skills training.
- Motivational and cognitive behavioural therapy short courses.
- Clinical Examination and Diagnostics module.
- Significant post registration experience in cardiology or cardiothoracic nursing.
- Experience with Cardiac Rehabilitation patients.
- Ability to revalidate as required by the NMC.
