The Community Nursing Service within North East Lincolnshire providenursing care and support to patients, families and carers in a variety ofcommunity settings supporting patients to remain in their own homes, preventinghospital admission, facilitation of early hospital discharges and managing endof life care.

The Community Nursing Service works closely and in partnership withPrimary Care Networks, Secondary Care, Social Care, Specialist Nurses and otherhealthcare staff to provide a quality service personalised to meet individualneeds. The service supports and assistspatients to manage ill health and long term conditions including elements ofdisability, thus allowing them to have the best possible quality of life, whilemaintaining dignity and respect.

Weare looking to appoint a qualified Registered Adult Nurse 1st Level withadditional post registration nursing qualifications at Advanced Practice MSclevel including V300 Independent prescribing and Long Term Conditions, alongwith community experience.

Main duties of the job

Community AdvancedPractitioner

Cleethorpes Primary Care Centre

24 hours a week - you will be required to work a rota pattern including weekends/bank holidaysbetween 08:00and 16:30

As Community Advanced Practitioner you willbe part of a highly experienced team of nurses who work closely with patientswith a chronic long term unstable conditions.

You will have a variety of tasks and responsibilities dependent on patientneed, including:

Carrying out physicalexaminations

Deciding on andcarrying out treatment, including prescribing of medicines, or referringpatients to an appropriate specialist

Using expertknowledge and clinical judgement to identify potential diagnosis and make a final diagnosis

Using extensivepractice experience to plan and provide skilled, competent care that meetspatients health and social care needs, involving thehealthcare team as appropriate

Ensuring the provisionof continuity of care, including follow-up visits

Assessing andevaluating, with patients, the effectiveness of the treatment and care providedand make changes as needed.

Liaise with health professionals to offer additional help and expertise with patientconsent.

Prescribe medicines,or liaise with GPs regarding medication management

Person Specification Experience
  • Experience in Long-term conditions for example, respiratory, heart disease, diabetes MND etc.
  • Experience of palliative care, wound management, continence care
  • Experience of Working within MDT
  • Case load profiling to ensure needs of local community are met
  • Experience in supervising and delegating tasks to others
  • Demonstrates experience working in the community
  • Experience of working and contributing within a case load of patients with complex care needs
Qualifications
  • Registered adult nurse (degree equivalent)
  • Degree/MSc in specialist practitioner Specialist Community Practitioner (DN) degree Autonomous/Advanced Clinical Practice
  • Independent Prescribing or a willingness to work towards this
  • Teaching qualification
  • QCF assessor
  • Motivational Interviewing experience
  • Health Coaching qualification
Skills & Knowledge
  • Specialist knowledge across a range of nursing procedures and practice underpinned by theoretical knowledge/relevant practical experience
  • Ability to work to deadlines and set priorities
  • Knowledge and application of current best practice in community nursing
  • Ability to manage the effective utilisation of resources e.g. Staff
  • Ability to develop and participate in nurse led clinics e.g. Continence, Doppler
  • Proven communications skills
  • Able to provide complicated and sensitive information for written reports/databases, and to complete patient records systems in operation e.g. SystmOne and other local systems.
  • Ability to communicate information to patients of varying needs e.g. hearing or visual impaired, learning difficulty etc.
  • Able to use negotiating, persuasive and influencing skills, maintaining confidentiality by participating in case to enhance multidisciplinary working.
  • Willingness to participate in research projects
