Join the EAST:MK Primary Care Network as a full-time clinical pharmacist, collaborating with our multidisciplinary team across four surgeries. Your role involves ensuring safe, evidence-based, and cost-effective prescribing, alongside offering support on medication queries and conducting medicines reconciliation.

• Work within a patient-facing role in a multidisciplinary team
• Manage areas of chronic disease management proactively
• Support delivery of the Network Contract Directed Enhanced Service
• Conduct structured medication reviews for patients with complex polypharmacy
• Provide clinical medicines expertise to patients and practice teams
• Improve prescribing practices and service quality through clinical audits
• Interface with community and hospital colleagues to resolve medication-related issues
• Assist in delivering enhanced health-in-care-homes initiatives
