Job Description

Working hours: full time (part timeapplicants will also be considered)

Annual Salary: dependent on experience

Central Surgery is based in Barton-upon-Humber, with a branch site in the village of Goxhill.

We are looking to recruit a clinical pharmacist to join our growing MDT team. Ideally candidates will hold an independent prescribing qualification, but we will support the successful candidate in achieving this qualification.

The clinical pharmacist is a patient-facing roleand there will be plenty of opportunity to utilise your clinical skills toimprove the health of the local community.

Main duties of the job

Thesuccessful candidate will work autonomously within the practice, but also aspart of the surgery team, to deliver the safe and effective use ofmedicines through managing long-term conditions, supporting hospital dischargeprescribing arrangements, providing specific advice for those on multiplemedications.

Youwill act as a specialist resource to answer medicines-related queries, applyingyour clinical and pharmaceutical skills to a variety of tasks within themedicines optimisation agenda. You will work alongside our highly valuedexisting practice and network clinical pharmacists as the role develops to meetthe NHS enhanced service requirements.

Thesuccessful candidate should be enthusiastic and adaptable to change, withexcellent clinical skills and a good level of competence in the use of IT. Goodorganisational skills and the ability to communicate effectively with a widerange of healthcare professionals and other stakeholders including patients,families and carers is an advantage.

About us

We are a large semi-rural dispensing practice, with 2 sites, serving our local community of approximately 17000 patient.

Rated as good by the CQC, we consider our surgeryto be dynamic, forward thinking practice. Advancing in times of change we operate a total triage system led by GP's.

Working with the PCN, we provide care aligned toboth national and local enhanced services, achieving high results for both QOFand IIF.

We are looking for a clinical pharmacist to joinour multi-disciplinary team; someone who will help deliver a high level ofpatient care and be a key member of staff, undertaking both clinical andeducational duties if needed.

Job responsibilities

The post holderwill be supported by existing clinical pharmacists employed within the practice and a GP clinical mentor.

You will be expected to lead in developing, promoting and implementing the high quality, evidencebased and cost effective use of medicines in primary care in a way thatmaximises the benefits of medicines to patients and which minimises risks(clinical, legal and financial).

The post holderwill also be required to develop, support and implement agreed prescribing andpharmaceutical activities or programmes within the practice.

Key Responsibilities

Patient-Facing Clinical Medication Reviews.

Undertake clinicalmedication reviews with patients (telephone and face to face), working within your scope of practice as anon-medical prescriber, implementing any necessary changes. The reviews could becohort based, in care homes, polypharmacy or any other area as required, within your level of competence.

To participate in, andcoordinate medicines optimisation services and clinical pharmacy development, to improve quality of care, personalised care and patientexperience. Provide clinical leadership to ensure high quality and costeffective prescribing.

Person Specification Experience
  • Understanding of the clinical pharmacist role within primary care
  • Demonstrates the ability to communicate complex and sensitive information in an understandable form
  • Work effectively independently and as a team member
  • An understanding of primary care prescribing.
  • Experience of working in Primary Care
  • Experience in the use of various IT applications and the recording of clinical notes (SystmOne, AccuRx)
Qualifications
  • Completed CPPE primary care pathway
  • Two years post qualification experience
