WestbourneMedical Centre are currently recruiting for a Clinical Pharmacist (ARRS funded role) to helpdeliver our vision of high-quality medical services at our well-established,progressive, friendly practice team in North Ormesby for 3 to 4 days per week.

Main duties of the jobWorkclosely with the GP's/ NP and Nurse to resolve day to day medicine issues.
Carryout telephone appointments with patients regarding medication management.
Towork as part of the multi-disciplinary team in a patient facing role toclinically assess and treat patients using their expert knowledge of medicinesfor specific disease areas.
Developa relationship with other pharmacy professionals across the PCN.
Bea prescriber or completing training to become a prescriber.
Beresponsible for the care management of patients with long term chronic diseasesand undertake clinical reviews ensuring they get the best use of theirmedicines and making the appropriate recommendations to GPs for medicineimprovement.
Answerrelevant medicine related enquiries from GP's, other practice staff, otherhealthcare professionals and patients.
Managemedicines from hospital discharges, intermediate care and into care homesincluding identifying and rectifying unexplained changes and ensuring patientsreceive the medicines needed post discharge.
Contributeto the achievement of QOF.
Undertakeclinical audits when required.
Carryout medicine optimisations and medicine prescriptions.

Provideleadership on person centred medicines optimisation and quality improvement,contributing to QOF and enhanced services.
Managingminor and common ailments under levels of competence, signposting to pharmacy, orreferring to GP when appropriate
Promoteselfcare to improve medicine wastage.
Answerrelevant medicine related enquiries from GP's, other practice staff, otherhealthcare professionals and patients.

Takea central role in the clinical aspects of shared care protocols, liaise withspecialist and community pharmacists.

Carryout medicine optimisations and medicine prescriptions.
Bepart of professional clinical network and have access to clinical supervisionincluding access to an assigned GP clinical supervision for support
Participatein practice meetings when requested.
Meetall deadlines for audits and written returns so the practice meets qualitystandards and receives the designated funding such as QOF,LES,DES, IIF.
Attainrelevant training to their position or training organised by the practice(clinical and non-clinical).
Toshow commitments to lifelong learning and audit to ensure evidence based bestpractice.
Contributeto the summarising of patient records and read coding patient data.
Showcompliance to and awareness of all relevant policies/guidelines including butnot limited to prescribing, confidentiality, data protection, health andsafety, QOF, CQC.
Person Specification Experience- Minimum of 2 years post qualification experience
- Excellent interpersonal skills.
- Excellent written and verbal communication skills.
- Can plan, manage, monitor, advise and review general medicine optimisation in core areas for long term conditions.
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
- Able to work under pressure and to meet deadlines and produce timely and informative reports.
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
- Experience in working in Primary care or GP practice
- Master's degree in pharmacy MPharm or equivalent
- Enrolled in or completed the CPPE Clinical Pharmacist training to work in primary care
- Mandatory registration with General Pharmaceutical Council
- DBS approved.
- Membership of the Royal Pharmaceutical Society
- A member of or working towards Faculty membership of the Royal Pharmaceutical Society
- Specialist knowledge acquired through postgraduate diploma level or equivalent training or experience.
- Independent prescriber
