Clinical Pharmacist

Stevenage Healthcare Ltd are supporting Stevenage South PCN to recruit an experienced clinical pharmacist to support the practices on a part-time basis. This position is offered on a part-time basis for one day a week - Friday 9am - 5pm. Working as an independent prescriber, the successful applicant willsupport the multi-disciplinary team, in line with the strategic objectives of the PCN.
Main duties of the job- Implementing an effective medicine management system within the practices, including cost-effective prescribing.
- Providing prescribing advice and support for both patients and colleagues.
- Undertaking clinical medication reviews with patients.
- Managing caseload of patients with common/minor/self-limiting ailments.
- Reconciling medicines following discharge from hospitals, intermediate care and into care homes.
Stevenage Healthcare Ltd is a new company which has been tasked with providing delivery and support for Stevenage South Primary Care Network. The company will provide development and delivery of SSPCN contracts and areas of work, as well as explore wider healthcare opportunities to support local people and provide positive health outcomes for our population.

Person Specification Experience- Ability to plan, organise and co-ordinate work effectively under their own initiative
- Ability to interpret, understand and analyse financial information
- Ability to plan and manage multiple workload priorities and meet deadlines
- Effective time management and skills in prioritisation
- Relevant experience of general practice IT systems
- Driving license and access to own vehicle
- Previously worked within a similar position within the public sector desirable.
- Completion of a degree in pharmacy and registration with the General Pharmaceutical Council.
