What You'll Do:
As a Clinical Pharmacist (Independent Prescriber), you'll work directly with GP practices and PCNs to:
- Optimise medication use and outcomes in long-term conditions (asthma, hypertension, diabetes)
- Conduct structured medication reviews and manage repeat prescriptions
- Provide expert advice on acute requests, discharge summaries, and clinical letters
- Support QOF and IIF performance
- Empower patients through education and guidance
Essential Criteria:
- GPhC-registered Pharmacist with Independent Prescribing qualification
- Clinical experience managing asthma, hypertension or diabetes
