About the post

• We are preferably looking for candidates with a particular interest in diabetes, however this is not essential.
• We would also ideally prefer candidates to have an independent prescribing qualification.
• Up to fulltime hours (37.5) but can be flexible by mutual agreement.
• We offer a competitive salary and package of benefits.
• In-house mentorship including appraisals + 1:1 meetings.
• Daily MDT coffee meetings
• Weekly clinical meetings

What a typical day looks like:

• Reviewing acute prescription requests
• Medicines Reconciliation of discharge and clinic letters
• LTC and medications reviews
• QOF, DES and IIF work

Our team consists of GPs, minor illness ANPs, an ANP specialising in care home/frailty and EOL patients, Practice pharmacists, Practice Nursing/HCA/Phlebotomy team and a strong management, administrative and reception team, as well as a strong and diverse allied health professional team via the PCN.

We are at the forefront of streamlining workload and improving patient care and have been recognised nationally for the work we have undertaken with NHSE regarding this.
