Job Description
The New Malden & Worcestor Park PCN have a excitingopportunity for a Clinical Pharmacist to join our dynamic multidisciplinaryteam. This patient-facing role is to enhance the quality of care for ourpatients.
The successful candidate will be provided with excellent clinicalsupport and an opportunity to develop their primary care skills.
We are looking for motivated, team-orientated individual whoshares our enthusiasm for improving patient care. Previous primary careexperience is preferred as well as excellent verbal communication skills, aproactive approach and a desire to learn.
As the role develops, you will takeresponsibility for areas of chronic disease management and undertake clinicalmedication reviews to proactively manage patients with complex poly-pharmacy.You will also provide primary support to general practice staff with regards toprescription and medication queries.

Main duties of the job The post holder will work as part of a multidisciplinaryteam to provide expertise in clinical medicines management, provide face toface structured medication reviews, manage long term conditions, management ofmedicines on transfer of care and systems for safer prescribing as well asmanage repeat prescription authorisation and reauthorisations.
Kingston GP Chambers has worked collaboratively with both secondary care and the private sector. At present we provide community clinics for Dermatology, Urology, Neurology, Dementia and Diabetes.
In addition, Kingston GP Chambers are at the forefront of improving patient access to primary care. We deliver a comprehensive extended hours service offering GP appointments across three hubs in the evenings Monday to Friday, over the weekend and on Bank Holidays.Kingston GP Chambers has the ability to respond rapidly, and we have continued to demonstrate the capability to provide high-quality, innovative and timely solutions to improving patient care and experience.
We work closely alongside the other GP Federations in Southwest London to continue the government's five-year forward plan and deliver patient services in the community.
We aim to continue to grow as we are increasingly recognised as the provider of choice for extended, enhanced and urgent GP services. We also expect an increasing role as we support, reinforce and develop the position of general practice as the core building block for an increasing range of jointly-provided services.

Job responsibilities Patient facing long term condition clinics
- Carry outconsultations with patients with single or multiple medical conditions wheremedicine optimisation is preferred
- Review the on-going need for each medicine, a review ofmonitoring needs and an opportunity to support patients with their medicinetaking ensuring they get the best use of their medicines.
- Make appropriaterecommendations to GPs for medicines improvement.
- Undertakeclinical medication reviews with patients and produce recommendations for GPson prescribing/monitoring.
- Undertakeclinical medication reviews with patients and produce recommendations for GPson prescribing and monitoring.
- Work with care home staff to improve safety andappropriate medication ordering (medication waste)
- Undertake clinical medication reviews with patients and produce recommendationsfor the senior clinical pharmacist, nurses and GPs on prescribing andmonitoring.
- Attend and refer patients to multidisciplinary case conference
- Managingcaseload of patients with common/minor/self-limiting ailments while workingwithin scope of practice and limits of competence.
- Sign posting to communitypharmacy and referring to appropriate healthcare professional i.e., GP/PracticeNurse/ Advanced Nurse Practitioner
- Telephone medication support provide telephone support forpatients with queries or concerns about their medication
- Management of medications at discharge from hospital toreconcile medications upon discharge from hospitals, immediate care and intocare homes including identifying and rectifying unexplained changes and workingwith patients and community pharmacists to ensure patients receive themedicines they need post discharge.
- Set up and manage systems to ensurecontinuity of medication supplies to high-risk groups of patients.
- Answer relevant medication queries from GPs, other network staff, otherhealthcare teams (e.g., community pharmacy) and patients with queries aboutmedications.
- Suggest and recommend solutions. Provide follow up with patientsto monitor the effect of any changes
- Review the use of medicinesmost commonly associated with unplanned hospital admissions and readmissionsthrough audit and individual patient reviews.
- Put in place changes to reducethe prescribing of these medicines to high-risk patient groups.
- Signposting ensure that patients are referred to theappropriate healthcare professional for the appropriate level of care within anappropriate period of time e.g., pathology results, common/minor ailments,acute conditions, long term condition review etc.
- Adhere and work within each networksrepeat prescribing policy. Manage the repeat prescribing and re-authorisationprocess by reviewing patient requests for repeat medications and reviewmedication review dates and arranging a review for these patients.
- Ensurepatients have the appropriate monitoring tests in place when required.
- Risk stratification identification of cohorts of patientsat high risk of harm from medicines through pre-prepared practice computersearches.
- Contribute pharmaceutical advice forthe development and implementation of new services that have medicinalcomponents (e.g., advice on treatment pathways and patient informationleaflets)
- Analyse, interpret and presentmedicines data to highlight issues and risks to support decision making.
- Undertake clinical audits ofprescribing in areas directed by the GPs, feedback the results and implementchanges in conjunction with the relevant practice team.
- Implement changes to medicines thatresult from MHRA drug safety alerts.
- Implementation of local and national guidelines andformulary recommendations - monitor practice prescribing against the local andnational formulary.
- Make recommendations to GPs for medicines that should beprescribed by hospital doctors or subject to shared care.
- To work with localmedicines management team to ensure practice compliance to local formulary.
- Provide education and training toprimary healthcare team on therapeutic and medicines optimisation
Work with the general practiceteam to ensure the practice is compliant with CQC standards where medicines areinvolved
Public Health
- To support public health campaigns. Toprovide specialist knowledge on all public health programmes available to thegeneral public.
- Collaborative working arrangements
- Participate in the PCN MDT meetings.
- Mandatory registration with General Pharmaceutical Council (GPhC).
- Independent prescriber and completion of CPPE.
- Independent prescriber or working towards/intent of gaining an independent prescribing qualification;
- Member of the Royal Pharmaceutical Society; Hospital or Primary Care experience; In-depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
- Minimum of 2 years experience as a pharmacist.
- Have some experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
- Recognises priorities when problem-solving and identifies deviations from normal pattern and can refer to seniors or GPs when appropriate.
- Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
- Good IT Skills.
- Work under pressure;
- Desirable - experience with using EMIS Good written and oral communication skills.
- Understanding of the nature of GP and General Practice.
- Understands and demonstrates the characteristics of a role model to members in the team and/or service.
