Clinical Pharmacist - Canalside Primary Care Network

This is an opportunity for a clinical pharmacist to join the team at Canalside Primary Care Network, working under the supervision of the wider clinical team. You will be joining the team currently consisting of 1 Pharmacists who is the Lead Pharmacist for the PCN.
Primary Care Networks are an opportunity for doctors practices within a locality to work together to deliver services at scale and to employ staff that support a larger service model than is possible at individual practices. Come and join us if you are looking to develop your skills as a pharmacist in a general practice setting, including medication review and reconciliation, medicine optimisation, and taking responsibility for areas of service improvement such as in polypharmacy.
Please note this role is based across the network, rather than just at Castleton Health Centre. You could be located in different practices on different days.

Main duties of the jobSet up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (eg, those with medicine compliance aids or those in care homes)
Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both
Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews
Put in place changes to reduce the prescribing of these medicines to high-risk patient groups
Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review to the GP. Monitor and manage patients with polypharmacy
Provide a telephone help line for patients with questions, queries and concerns about their medicines
Undertake clinical medication reviews with patients and produce recommendations for the GP on prescribing and monitoring
Work with care home staff to improve safety of medicines ordering and administration

The host organisation for the role is Castleton Health Centre who will be your employer whilst you work across the PCN.
Job responsibilitiesTo reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g., those with medicine compliance aids or those in care homes).

Produce and implement a repeat prescribing policy.
Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review to the GP. Monitor and manage patients with polypharmacy.
Provide a telephone help line for patients with questions, queries and concerns about their medicines.
Hold clinics for patients requiring face-to-face medicines use reviews
i.e., advise about medicines and adherence support.
Undertake clinical medication reviews with patients and produce recommendations for the GPs on prescribing and monitoring.
Undertake clinical medication reviews with patients and produce recommendations for the GP on prescribing and monitoring.
Work with care home staff to improve safety of medicines ordering and administration.
See patients with single medical problems where medicine optimisation is required (e.g., COPD, asthma).
Make recommendations to GPs for medicine improvements.
Contribute pharmaceutical advice for the development and
implementation of new services that have medicinal components (e.g., advise on treatment pathways and patient information leaflets).
Person Specification Experience- Working in primary care
- GPhC registration assessment with a pass result
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
