Care Team Leader

Rivermead Residential Care Home in Kempston is looking for a Care Team Leader to join their team.

Company: Bedford Borough Council, Location: Kempston, Bedford, MK43 0GZ.

The successful candidate will be able to liaise with external professionals, have experience working with Dementia, have worked in a care setting for at least 2 years, and be able to administer medication.

Responsibilities

• Liaise effectively with residents and/or their representatives, and with internal and external professionals and contractors as required.
• Report and record accurately appropriate information relating to residents, staff, and the operation of the home.
• Administer medication in accordance with policy, ensuring residents' care needs are regularly assessed and supported by individual personal care plans and risk management assessments with clear interventions, and that these are reviewed monthly or more frequently if changes occur.
• Ensure services are delivered, maintained, and in accordance with legislative and statutory requirements, policies, and procedures of the organisation.

Qualifications

• At least 2 years of experience working in a care setting.
• Experience working with Dementia.
• Ability to administer medication.
• Strong liaison and communication skills with external professionals.
