ROLE OVERVIEW Full Time Care Client Liaison Lead -Job role Description - What the Role InvolvesBurley's Home Care is looking for a full-time Care Client Liaison lead for our Live-in-Care Packages. We want someone who is passionate about helping others. This job is based in Ringwood, our clients are based in Dorset, Hampshire, and Wiltshire. You will work closely with our existing Live-in Care and Domiciliary service users, as well as developing new Live-in-Care leads and packages. Liaising with the service users and their families, and our care teams. Core duties and responsibilities include: Mentoring and support: Act as first point of contact for new Live-in-Carers and liaising with our existing established Live-in-Carer's community. Providing encouragement and practical guidance, support, introductions, solving issues, ensuring a seamless and smooth handover with our clients. Liaising with the Recruitment & Training Officer - to ensure there is a suitable pipeline of quality Live-in Carers. Great communication with our in house Recruiter is a vital part of this role, to ensure dates, times, locations, and the service users care information are provided in detail, to ensure the best match for our clients. Care coordination and case management: Highly organised, with a positive and friendly attitude. Align carer skills with the needs of service users. Assisting with care plans, scheduling and covering our Domiciliary Care Coordinator, where necessary. To be part of the on-call Rota team. Documentation: Accurately records keeping, confident in using electronic care platforms, someone who picks up new systems fast, and can adapt to every changing requirement. Client relationships: Work closely with service users and families, conducting assessments, reviewing care plans, and delivering excellent customer service is key. Liaising closely with the Registered Manager. Quality assurance: Conduct site visits and supervision, picking up the Live-in-Carers from the train locally and taking to the Live-in-Care Package location.
