Cardiac Rehabilitation Practitioner (Band 6)

Make a meaningful difference to the lives of people living with heart failure by delivering high quality, evidence based cardiac rehabilitation services across community and clinical settings.
You will lead and support Phase 3 heart failure rehabilitation classes, work directly with patients in their own homes and contribute to optimal symptom management and the maximisation of medical therapy. Your role includes specialist assessment, advice, rehabilitation and ongoing support, ensuring care aligns with NICE guidance, BACPR standards, and local policies.
Responsibilities- Deliver timely, high quality cardiac rehabilitation in line with clinical protocols, service priorities and professional standards.
- Undertake comprehensive specialist assessments using investigative and analytical skills.
- Plan, deliver and evaluate personalised cardiac rehabilitation interventions in partnership with patients, carers and MDT colleagues.
- Provide care flexibly in clinic and community environments.
- Promote exercise, self management and lifestyle change to support long term health outcomes.
- Monitor outcomes and adapt interventions to optimise patient progress and experience.
- Give advice, education and instruction to patients, carers, students and other health professionals.
- Communicate effectively via methods and systems (including SystmOne and digital platforms), adapting to overcome barriers and using interpreting services where required.
- Work within clinical supervision frameworks, escalating complex cases with the Clinical Lead.
- Contribute to service planning, development and quality improvement initiatives.
- Lead on delegated responsibilities and service development projects.
- Communicate effectively with internal teams and external partners across health, social care and voluntary sectors.
- Maintain competence in the safe use and storage of equipment, following Trust policies.
- £39,959 - £48,117 FTE (Band 6 AfC Salary) with NHS Pension and full Agenda for Change terms and conditions.

- Current registration with a relevant healthcare professional body.
- Minimum of two years post registration experience within cardiology and/or cardiothoracic services.
- Experience delivering education or teaching to patients, carers, students or colleagues.
- Experience evaluating and contributing to the improvement of clinical care.
- Sound knowledge of national policies and guidance influencing service delivery (e.g. NICE).
- Strong interpersonal and communication skills.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Awareness of research principles and evidence based practice.
- Full, valid UK driving licence.
- Access to a road worthy, insured and MOT compliant vehicle.
- Educated to, or currently working towards, a master's level qualification.
- Experience supporting or delivering health promotion activities.
