Job Description
Band 6 - Community Psychiatric Nurse (CPN)

Team: Referral Management Centre Location: Birmingham Band: 6

Rate - £27 Umbrella ph

Role Summary:

The Band 6 CPN will work within the Referral Management Centre to support children and young people referred to the service. The role combines screening referrals, conducting assessments, and coordinating care within a multidisciplinary team framework. The nurse will also undertake shift co-ordinator responsibilities for one day per week.

Key Responsibilities:
  • Referral Screening & Triage: Identify urgent, crisis, and routine cases; take calls from professionals, parents, or guardians regarding potential mental health concerns.
  • Initial Assessments: Conduct face-to-face assessments to determine the needs of children and young people.
  • Multidisciplinary Team Meetings: Attend daily MDT meetings to discuss outcomes, agree on interventions, and coordinate care plans.
  • Care Planning & Intervention: Develop individualised care plans, contribute to decision-making, and monitor treatment pathways.
  • Risk & Crisis Management: Assess and manage risks, safeguarding concerns, and provide crisis interventions where required.
  • Shift Coordination: Lead referral screening and allocation for one day per week.
  • Supervision & Mentorship: Support junior colleagues and students, promoting learning and professional development.
  • Documentation: Maintain high standards of clinical records in line with operational frameworks and service guidance.
Person Specification / Skills Required:
  • Registered Mental Health Nurse (RMN) with community or CAMHS experience.
  • Experience in mental health assessment, risk management, and crisis intervention.
  • Strong ability to work collaboratively in a multidisciplinary team.
  • Excellent communication, organisational, and documentation skills.
  • Confidence in IT and clinical systems relevant to community mental health services.
  • Ability to mentor and supervise junior colleagues.
