What you will do:
  • Conduct thorough assessments, planning, implementation, and evaluation of evidence-based care for service users.
  • Perform clinical risk assessments and make informed decisions for those contacting the "Out of Hours" service.
  • Collaborate on admissions, case conferences, and discharge planning with the multidisciplinary team.
  • Coordinate Care Programme Approach assessments and manage a designated caseload.
  • Assist in service development by setting quality standards and conducting audits and reviews.
  • Fulfill professional responsibilities under Safeguarding Vulnerable Adults and Children Legislation.
  • Comply with the Mental Health Act 1983's requirements and facilitate carer and service user involvement.
  • Establish therapeutic relationships and implement evidence-based interventions.
  • Deliver carers' assessments, education, and support in line with National Service Framework requirements.
  • Provide coverage and support for colleagues as needed.
