The Elmdon Community Unit provides highquality care focused on rehabilitation, recovery, and safe, timely discharge. As a Flow Coordinator, you will play a vital role in maintaining safe, effective flow throughout the unit. Working closely with nursing, therapy, discharge and clinical teams, you will be responsible for ensuring that pathways run smoothly from admission through to discharge.

Main duties of the job

• Proactively monitor and coordinate daily patient flow, capacity, and bed utilisation across the unit
• Support safe and timely discharge planning in collaboration with the wider multidisciplinary team
• Maintain clear, accurate documentation and flow dashboards, escalating risks where required
• Liaise with internal and external partners including Integrated Care Teams, community services, and therapy teams
• Support operational meetings, huddles, and flow discussions to ensure seamless communication
• Contribute to continuous improvement activities to enhance patient experience and operational efficiency

Person Specification Qualifications

• Good standard of general education to Level
• 2 standard including Maths / English
• Qualifications to level 2 / GCSE or equivalent
• Level 3 Qualification ( A level / AS / BTEC /
• NVQ in a related subject to business
• administration or equivalent comparable
• experience

Experience

• Experience in the provision of administrative
• support to the management of patients /or
• patient facing role in an NHS provider
• organisation
• Experience of Multi-disciplinary working /
• liaison within an acute NHS provider
• organization
• Experience in being the point of contact for
• enquiries from external / internal users /
• patients and families
• Can demonstrate the use of judgment and
• assessment of situations where there is more
• than one solution to an issue
• Can demonstrate ability to deal with routine
• and non -routine and unpredictable
• Understanding of the scope of the role in
• context of the team and the organisation,
• and how the role may contribute to service
• development
• Knowledge of administrative procedures
• including NHS IT systems which contain

• patient flow within an acute hospital setting
