Band 4 Flow Co Ordinator - Elmdon Unit

The Elmdon Community Unit provides highquality care focused on rehabilitation, recovery, and safe, timely discharge. As a Flow Coordinator, you will play a vital role in maintaining safe, effective flow throughout the unit. Working closely with nursing, therapy, discharge and clinical teams, you will be responsible for ensuring that pathways run smoothly from admission through to discharge.

Main duties of the job
  • Proactively monitor and coordinate daily patient flow, capacity, and bed utilisation across the unit
  • Support safe and timely discharge planning in collaboration with the wider multidisciplinary team
  • Maintain clear, accurate documentation and flow dashboards, escalating risks where required
  • Liaise with internal and external partners including Integrated Care Teams, community services, and therapy teams
  • Support operational meetings, huddles, and flow discussions to ensure seamless communication
  • Contribute to continuous improvement activities to enhance patient experience and operational efficiency

  • Experience in the provision of administrative
