Assistant Care Manager
Posted 6 days 7 hours ago by Connect 2 Flexicare
Are you a compassionate, organised care professional ready to step up into a leadership role? Connect2Flexicare is a growing domiciliary care provider based in Aylesbury, and we're looking for an Assistant Care Manager to help us deliver outstanding, person-centred care across our community.
Do you find yourself being the person colleagues turn to when something tricky comes up? Have you ever looked at a care plan and thought, "I could make this so much better"? Are you ready to stop just delivering care and start shaping how it's delivered? If that sounds like you, keep reading.
This is a hands on role for someone who thrives on variety. One day you're refining a care plan, the next you're supporting a carer in the field, liaising with a social worker, or helping a family navigate a difficult moment. If you care deeply about doing things properly and want to grow into a senior management role, we want to hear from you.
What You'll Be DoingCare Planning & Clinical Oversight - You'll develop, review, and update care plans to ensure every service user receives care that genuinely fits their needs. You'll manage medication oversight, including MAR chart updates and medication checks, carry out risk assessments, and conduct spot checks and client visits to make sure the care being delivered is safe, dignified, and compliant.
Supervision & Staff Management - You'll schedule and oversee field staff coverage, provide day to day supervision and support to our carers, and follow up on performance through spot checks and one to ones. You'll also share the on call rota, providing operational support roughly once every three weeks.
Incident & Safeguarding Management - When things need attention, you'll be the steady hand, logging and investigating incidents, managing safeguarding concerns promptly, and liaising with the local authority and other professionals. Accurate, timely documentation will be second nature to you.
Communication & Stakeholder Engagement - You'll be a key point of contact for service users, families, district nurses, social workers, and other professionals. You'll attend team and professional meetings and make sure client requests, from equipment needs to care adjustments, are followed through.
Administration & Compliance - You'll keep case notes, logs, and reports accurate and up to date, support audits and quality checks, and help ensure we're always ready for CQC inspection.
What We're Looking For- Previous experience in domiciliary or community care, ideally in a senior carer, team leader, or coordinator role
- Working knowledge of CQC fundamental standards, safeguarding, and care planning
- NVQ Level 3 in Health and Social Care, or working towards it
- Confident communicator, both with vulnerable people and with external professionals
- Highly organised, calm under pressure, and able to juggle competing priorities
- Comfortable with care management software and general IT
- Full UK driving licence and access to your own vehicle
- Enhanced DBS, or willingness to undergo one
- Real career progression - this role is a genuine stepping stone to Care Manager and beyond, not a dead end title
- Funded training and qualifications, including support to complete your Level 4/5 if you want it
- A seat at the table - we're a family run business, which means your ideas actually get heard and acted on, not buried in a corporate inbox
- Variety every day, no two days look the same, and you'll never be stuck behind a desk
- Work that matters, you'll see the direct impact of your decisions on real people's lives, every single week
- A supportive team - we look after our staff the same way we expect them to look after our clients
