ANP - Advanced Nurse Practitioner

We are looking for a an experienced qualified ANP to join our friendly nursing team.
This is a part-time role but we would consider full-time.
Experience of working in general practice is essential.
6 weeks annual leave.
Main duties of the jobWorking closely with the GPs, the ANP will provideadvanced, specialist nursing care to patients and manage clinical areas, including chronic disease clinics.
About usWe have 5 GP Partners and are based over two sites, Greenside and Country Park and part of the Croydon GP Super Network PCN.

Job responsibilitiesManagepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice
Implementand evaluate individual specialised treatment plans for chronic diseasepatients
Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required
Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health
Provideadvanced, specialist nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF
Providewound care, i.e. ulcer/Doppler
Providespecialist clinics such as necessary
Developand implement health promotion and wellbeing programmes
Undertakethe collection of pathological specimens
Requestpathology services as necessary
Processand interpret pathology and other test results as required
Maintainaccurate clinical records in conjunction with extant legislation
Ensureaccurate read-coding on the clinical system
Maintainchronic disease registers
Developand implement an effective call/recall system
Assistin the implementation of an effective immunisation programme
Implementvaccination programmes for adults and children
Bean extended and supplementary prescriber, adhering to extant guidance
Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice), reviewing annually as required
Contributeto practice targets including QOF, locally commissioned services, complying with local and regional guidance
Liaisewith external services/agencies to ensure the patient is supportedappropriately
Delegateclinical responsibilities appropriately, ensuring safe practice and that thetask is within the scope of practice of the individual
Supportthe clinical team with all safeguarding matters in accordance with local andnational policies
Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
Deliveropportunistic health promotion where appropriate
Person Specification Qualifications- Registered Nurse with Nursing and Midwifery Council
- Meets the standards for registered ANP working at advanced level
- Qualified Independent Nurse Prescriber on the NMC register
- Meets NMC revalidation requirements
- Experience of prescribing and undertaking medication reviews
- Experience of working as a practice nurse or community nurse
- Experience of working in a primary care environment
- Evidence of continuing professional development (CPD) commensurate with the role of an ANP
- Understand the importance of evidence-based practice
- Broad knowledge of clinical governance
- Ability to record accurate clinical notes
- Ability to work within own scope of practice and understand when to refer to GPs
- Knowledge of health promotion strategies
- Understand the requirement for PGDs and associated policy
- Polite and confident, flexible and cooperative
- Motivated, forward thinker
- Problem solver with the ability to process information accurately and effectively, interpreting data as required
- High levels of integrity and loyalty
- Meet the requirements and produce evidence for nurse revalidation
- Sensitive and empathetic in distressing situations as well as ability to work under pressure/in stressful situations
