Agency Social Worker - Reablement Service - Brent Council (up to 12 month assignment)

Our client, Brent Council, is seeking an agency Social Worker for its Reablement service.

This role will last 6 months initially but more likely up to 12 months and is paid at £32 per hour.

The roles will involve the following:

• An integrated service that delivers fast-paced responses to hospital discharges, alongside reablement and social care support through a short-term intervention model.
• Our team ethos centres on promoting and enabling independence, and we maintain strong working relationships with partners and colleagues across hubs and intermediate care services.
• Completing Care Act assessments
• Care and Support plans
• Multi-agency working
• Working in a fast-paced way, taking up to 3 allocations per week and actioning them quickly
• Visiting people out in the community - no telephone-based assessments
• Working closely with Reablement providers and getting feedback from them
• Working in a team which includes Occupational Therapists and OT assistants
• Liaising with GPs, physiotherapists etc.
• Using Mosaic and benefitting from the Magic Notes to lessen admin tasks

Requirements of the role:

• Qualified Social Worker
• Fully Social Work England registered
• Must have post-qualified experience working within a local authority adult social care setting
• Does not require specialist Reablement experience, but the ideal candidate will have worked with adults with physical disabilities and will have demonstrable experience in a fast-paced setting such as hospital discharge, front door team, duty etc.
• Comfortable being in the local area 3 days per week for visits and some office presence
