Job Description
Join Nottingham CityCare's highly regarded MOSAIC Service (Musculoskeletal, Orthopaedic, Spinal Pain and Integrated CFS) as an Advanced Practitioner Physiotherapist.
In this role, you'll deliver advanced MSK assessment, diagnosis, first-line treatment and decision-making, including investigations and referrals. You will contribute to improving patient outcomes, developing the service, and supporting junior clinicians. Opportunities include:

Clinical
In this role, you'll deliver advanced MSK assessment, diagnosis, first-line treatment and decision-making, including investigations and referrals. You will contribute to improving patient outcomes, developing the service, and supporting junior clinicians. Opportunities include:
- Orthopaedic triage of secondary-care referrals
- Clinical assessment and treatment planning within ICATS clinics
- Diagnostic referrals (MRI, X-ray, USS)
- Service evaluation and audit
- Regular supervision and structured in-service training
- Experienced clinicians able to work autonomously and manage complex decision-making across varied settings
- Band 6 MSK physiotherapists seeking career progression in a supportive MDT environment

Job responsibilities Job Purpose
- Provide clinical expertise undertaking the assessment, planning and development of treatment programmes for clients with a wide variety of complex physical needs in a clinic setting.
- Progress and request investigations to facilitate diagnosis and choice of treatment regime, understanding the information limitations derived from these and the relative sensitivity and specificity of particular tests diagnostic services such as x-rays and blood test, and interpret and act on results to aid diagnosis and the management plans of patients.
- Deliver programmes of supported patient self management, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions.
- Be responsible for continuing the on going development of high standards of clinical practice and expertise within the service, contributing to implementation of specific changes to practice and service protocols.
- To be responsible for the delivery of the MOSAIC service for clients referred by GPs, other health professionals, social services and self referral.
- To supervise less experienced physiotherapists, support staff, and students. To supervise, educate and assess the performance of undergraduate physiotherapy students as required, ensuring to the standard of practice and teaching meets the standards set by the degree level qualification.
- To monitor and maintain performance of clinical sub team of physiotherapists.
- To work in an extended scope including but not exclusive to:
- Orthopaedic triage of secondary care referrals
- Low Back Pain triage of secondary care referrals
- Clinical assessment and treatment planning in Integrated Clinical Assessment and Treatment (ICATS) clinics
- Referral for diagnostic examinations (MRI scan, x ray)
- Administering and evaluating outcome of injection therapy
Clinical
- To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care and to support more junior staff to do likewise.
- To undertake general office procedures including photocopying, faxing, and the distribution of mail as required.
- To work effectively as part of a team to provide cover for other administration staff when required and to be flexible regarding working hours to meet the needs of the service.
- To be customer focused on behalf of the service and ensure that the reception people are given is welcoming and helpful.
- To have a lead role and determine by assessment and liaison with other relevant agencies the advice, education and treatment requirements of each client.
- Accountable for decisions and actions via HCPC registration, supported by a professional culture of peer networking/review and engagement in evidence based practice.
- To undertake the comprehensive, advanced assessment of clients including those with complex and/or chronic presentation using investigative and analytical skills and to formulate the individualised management and treatment plans, using clinical reasoning, a wide range of treatment skills and options to devise a specialised, individualised programme of care.
- Streamlines pathway of care by providing a responsive service so that patients receive timely access to care.
- To accept clinical responsibility for a designated caseload of patients and to organise this effectively and efficiently, with regard to clinical priorities and time management.
- To provide highly specialised advice, teaching and training to other members of the primary health care team regarding physiotherapy management of clients with problems related to rehabilitation, ageing and obesity.
- To educate, supervise and manage less experienced members of the team along with support staff and students. Using appropriate documentation.
- To assess capacity, gain informed consent and work within a legal framework with clients who lack the capacity to make informed decisions.
- To provide spontaneous and planned advice, teaching and instruction to patients, carers and other professionals to promote an understanding of the aims of physiotherapy and ensure a consistent approach to client care.
- Uses a range of clinical skills which may include:
- non medical independent prescribing
- joint aspirations
- Exercises professional judgement, making, justifying and taking responsibility for decisions in unpredictable situations, including in the context of incomplete/contradictory information.
- Manages interactions in complex situations, including with individuals with particular psychosocial and mental health needs and with colleagues across the primary care team, sectors and setting.
- Works with MDT to develop more effective and streamlined clinical pathways and services.
- Provides learning opportunities for the whole multi professional team within primary care.
- To participate in the development of good practice for physiotherapists within Nottingham CityCare Partnerships under the guidance of the professional lead for physiotherapist.
- To participate in the services marketing strategy internally and externally, developing local and regional networks.
- Demonstrate a commitment to evidence based practice and make recommendations for change.
- To be responsible for your own continued professional development, attend training events, conferences and study days as highlighted within the organisation's training strategy and your own professional and personal development.
- To demonstrate sound knowledge of Clinical Governance and risk management and its application to the workplace.
- To participate in the staff appraisal scheme and personal development plans as both supervisor and supervisee.
