• Provide high-quality supervision and support to personal advisors working with care leavers.
• Collaborate with the Leaving Care Team to plan effective pathways and support for young people transitioning into adulthood.
• Develop and implement care and pathway plans tailored to the needs of each young person.
• Build and maintain strong relationships with partner agencies to enhance the support and services young people receive.
• Monitor and review the progress of each care leaver, adjusting support plans as necessary.
• Advocate for care leavers within the service and in external meetings, ensuring their voices are heard and needs met.
