Job Description

Advanced Nurse Practitioner/Advanced Paramedic Practitioner to join the triage team working up to three days/week, Wednesday to Friday.

Main duties of the job

TheAdvanced Clinical Nurse Practitioner/Paramedic Practitioner will:

  • Work as part of a multi-disciplinary team (MDT) within Bramblehaies Surgery and as part of the wider PCN clinical team.
  • Assess and triage patients including same day triage and, as appropriate, provide definitive treatment (including prescribing medications, following policy, patient group directives, national e.g. NICE and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
  • Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
  • Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
  • Actively take a personalised care approach and population-centred care approach to enable shared decision making with the presenting person.
  • Undertake home visits as required
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCN social prescribing service and, where appropriate, other community or voluntary services.

About us

Bramblehaies Surgery is a semi-rural practice in the heart of Devon serving a population of 7,500 patients, providing high standards of care (CQC rating of Good) from a partner owned modern, purpose-built building.

Our team currently consists of four GP partners, two salaried GPs, Nurse Practitioner, Practice Manager, Patient Services Manager, Data Quality Administrator, Clinical Pharmacist and Pharmacy Technician, a team of three Practice Nurses headed up by a Lead Nurse, three HCAs, a General Practice Assistant, a committed admin/reception team and our much-needed housekeeping team.

We are a member practice of Culm Valley Primary Care Network and supported with Social Prescribers, Health and Wellbeing Coach, First Contact Physio, Mental Health Worker, and a Care Home Team consisting of a Nurse Practitioner and an Occupational Therapist.

We are a training practice and a Visa-sponsoring practice hosting GP Registrars and medical students.

Job responsibilities

The Advanced Nurse Practitioner/Paramedic Practitioner will:

  1. Work as part of a multi-disciplinary team (MDT) within Bramblehaies Surgery and as part of the wider PCN clinical team.
  2. Assess and triage patients including same day triage and, as appropriate, provide definitive treatment (including prescribing medications, following policy, patient group directives, national e.g. NICE and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
  3. Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
  4. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
  5. Actively take a personalised care approach and population-centred care approach to enable shared decision making with the presenting person.
  6. Be able to perform specialist health checks and reviews within their scope of practice and in line with local and national guidance;
  7. Support the delivery of anticipatory care plans and provide an alternative model to urgent and same day GP home visit for the Practice/PCN.
  8. Complete the relevant training to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example Practice Nurses and the personalised care roles.
  9. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCN's social prescribing service and, where appropriate, other community or voluntary services.
  10. Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered.
  11. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
  12. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
  13. Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices.

Person Specification Experience
  • Minimum of 5 years' post registration
  • Experience of chronic disease management
  • Management of minor illnesses or ailments
  • Experience of prescribing and undertaking medication reviews
  • Experience of working in a GP practice
Qualifications
  • Registered on the relevant professional register NMC or HCPC
  • Hold a masters award and achieved all 4 pillars of advanced clinical practice, including a digital badge
  • Independent prescriber
  • Experience in a senior clinical nurse/paramedic role
