As an ACP at Lombard Medical Centre, you will be delivering high quality, person centred care across a blended role comprising both clinic based consultations and home visits. You will work autonomously, drawing on your advanced clinical assessment and diagnosticskills, and collaborate closely with our experienced multi professional team.

Main duties of the job

Your core responsibilities will include:

  • Assessing, diagnosing, planning, treating and reviewing patients with both acute and chronic conditions
  • Carrying out home visits and leading care for patients with learning disabilities
  • Managing clinical episodes independently including prescribing and referrals
  • Taking a lead in unscheduled and emergency care
  • Performing clinical procedures and planning care pathways
  • Communicating effectively with patients, carers and clinical teams

The Details

Hours: 37.5 hours/week (full time) or part time considered Salary: Agenda for Change Band 8a Location: Lombard Medical Centre, Nottingham Accountable to: Clinical Lead/Practice Manager Start date: Negotiable

Job responsibilities
  • Use highly developed clinical knowledge to independently assess, diagnose, plan, implement and evaluate treatments and interventions for patients presenting to Lombard Medical Centre frequently with complex undifferentiated or undefined presentations.
  • Undertakes physical examination as indicated by the patient's condition; autonomously analysing complex clinical signs and investigation results to consider differential diagnoses and diagnose conditions, initiating treatment as required.
  • Performs clinical procedures appropriate to the history and physical examination of patients, including further invasive testing and treatments requiring highly developed skills and precision.
  • Requests investigations such as blood, urine and other laboratory tests, electrocardiographs (ECGs), ultrasound scans, X Rays and computed tomography (CT) scans in accordance with IR(ME)R regulations and as local policy allows.
  • Analyses multiple sources of data including patient history, physical examination and investigation findings when making diagnoses, clinical judgements and evaluating care provided; presenting this information to GPs, other medical staff for advice when scope of practice is exceeded.
  • Autonomously formulates appropriate management plans for patients, formulates clinical decisions and often complex treatment plans to manage acute illness and acute episodes of chronic illness including referral, admission, or discharge from service.
  • Continuously re evaluates findings and clinical response to treatment and establishes an appropriate ongoing management plan accordingly.
  • Prioritises health problems and intervenes appropriately in complex, urgent and emergency situations, including initiation and leadership of resuscitation.
  • Manages clinical events involving patients, often requiring unpredictable and high levels of mental and/or physical effort according to the patient's dependency and clinical need.
  • Provides clinical cover to the surgery according to patient and staffing needs.
  • Prescribes and reviews medication (as an independent prescriber, if relevant) for therapeutic effectiveness appropriate to patient need and in accordance with best/evidence based practice and national and local protocols and within the role's scope of practice and legal framework. Integrates both pharmacological and non pharmacological treatment in patient care/management plans.
  • Assesses patients with mental health needs using local policy and guidelines and refers to the appropriate services.
  • Communicates effectively with patients, carers, colleagues, and others using appropriate communication styles. Anticipates barriers to communication and ensures patients and significant others are kept fully informed and consent to treatment.
Person Specification Analytical and judgement skills
  • Demonstrates:
  • Ability to respond to, prioritise and analyse complex health conditions and ensure that effective interventions are actioned in a safe and timely manner.
  • Ability to deal effectively with clinical issues within defined procedures and guidelines.
  • Ability to work with undifferentiated and undiagnosed healthcare problems.
  • Participation in research and audit projects.
  • Active involvement in organisational operational management.
  • Leadership in audit or research.
  • Involvement in the investigation of serious clinical incidents.
Qualifications
  • Current registration with the relevant professional body, such as NMC or HCPC.
  • MSc in advanced clinical practice or an equivalent MSc or NHSE Centre for Advancing Practice Digital Badge (or working towards it).
  • Registered Independent Prescriber.
  • Evidence of teaching, supervision, and assessment skills.
  • Mentor/assessor and/or teaching qualification.
  • NHSE Centre for Advancing Practice Digital Badge (or working towards it).
Experience
  • Currently working clinically in an advanced practice role.
  • Has an up to date portfolio of supervised practice in advanced clinical practice.
  • Has evidence of continued professional development (CPD) following successful completion of MSc & qualification as.
  • Management and leadership experience.
  • Evidence of involvement in policy and practice change.
  • Clinical work in an autonomous role.
