Advanced Nurse Practitioner (ANP) GP Surgery

We are seeking an experienced and highly motivated Advanced Nurse Practitioner (ANP) to join our primary care team for 3 days per week.

This is an exciting opportunity for a clinician with strong autonomous practice skills to work within a supportive, multidisciplinary environment. The successful candidate will provide high-quality, patient-centred care, including assessment, diagnosis, treatment, and management of both acute and chronic conditions.

Main duties of the job

The ANP will work independently and collaboratively to deliver a high standard of clinical care. Key responsibilities include:

  • Assess, diagnose, plan, implement, and evaluate treatment/interventions for patients presenting with undifferentiated and undiagnosed conditions
  • Manage acute presentations, minor illness, and long-term conditions
  • Prescribe medications in accordance with local and national guidelines (if qualified as an independent prescriber)
  • Order and interpret investigations such as blood tests, imaging, and ECGs
  • Work autonomously within scope of practice and professional guidelines
  • Provide health promotion, education, and preventative care
  • Contribute to practice development, clinical governance, and quality improvement initiatives
  • Support and mentor junior staff where appropriate
  • Maintain accurate and contemporaneous clinical records

Oakwood Medical Centre is a medical centre situated in the heart of Barnton, Northwich (Cheshire), with approximately 9,000 patients across a mixed rural and urban community.

  • 1 ANP, 1 Practice Nurse and 1 TNA
  • Experienced administration team
  • Shared roles across Northwich PCN, including Social Prescribers, Paramedics, and OTs

We are an active member of the Northwich Primary Care Network (PCN) and work closely with local providers to deliver innovative and integrated patient care.

As a practice, we are committed to service development and digital innovation, supporting the regional Digital & Data Strategy, including the introduction of clinician-facing decision tools and smarter ways of working.

We also collaborate with Cheshire & Merseyside Medicines Management teams to improve prescribing safety, consistency, and quality of care.

Oakwood Medical Centre operates a Total Triage model, and the successful candidate will be expected to work within this system.

Job responsibilities

The post holder will:

  • Deliver advanced clinical care in line with national competencies for ANPs
  • Work within the NMC Code and local clinical governance frameworks
  • Participate in clinical audit, significant event analysis, and service improvement
  • Maintain continued professional development and revalidation requirements
  • Work flexibly to meet service demand

Primary Key Responsibilities:

Clinical Practice

  • Provide advanced, autonomous clinical care including history taking, examination, diagnosis, and treatment planning
  • Manage patients presenting with undifferentiated and complex conditions across all age groups
  • Deliver care for both acute presentations and long-term conditions
  • Prescribe safely and effectively within scope of practice (if qualified as an Independent Prescriber)
  • Request, interpret, and act upon diagnostic investigations
  • Make appropriate referrals to secondary care and other services
  • Undertake home visits where appropriate

Clinical Leadership & Service Development

  • Contribute to the development and delivery of high-quality, safe, and effective services
  • Support implementation of new clinical pathways and service improvements
  • Participate in clinical governance, including audit and quality improvement initiatives
  • Act as a clinical role model within the multidisciplinary team
  • Contribute to the development of practice policies and protocols

Multidisciplinary Team Working

  • Work collaboratively with GPs, nurses, pharmacists, and wider PCN teams
  • Contribute to MDT meetings and care planning for complex patients
  • Support integrated working across primary, community, and secondary care
  • Support and mentor junior staff, students, or trainee ANPs where appropriate
  • Contribute to education within the practice team
  • Maintain own continuing professional development and revalidation

Patient Experience & Safety

  • Deliver patient-centred care that is responsive to individual needs
  • Promote health education, self-care, and prevention
  • Maintain accurate, clear, and contemporaneous clinical records
  • Ensure compliance with safeguarding, infection control, and safety policies

Digital & Operational Working

  • Work effectively within a Total Triage model
  • Utilise digital consultation tools and clinical systems (e.g. EMIS/SystmOne)
  • Support adoption of digital innovations within the practice and PCN
Person Specification Experience
  • Significant post-registration experience in primary care, urgent care, or similar setting
  • Experience managing undifferentiated and complex patients
  • Evidence of autonomous clinical decision-making
  • Experience of working in a primary care environment
  • Experience of continued professional development
  • Experience of QOF and clinical audit
  • Experience of chronic disease management
  • Experience of working with the PCN DES, NICE, and ARRS framework.
  • Experience in leadership, teaching, or mentoring roles
  • Additional relevant clinical qualifications (e.g. minor illness/injury)
  • Experience of medicines management
  • Experience of ICB initiatives
Qualifications
  • Registered Nurse with valid NMC registration
  • MSc in Advanced Clinical Practice
  • Independent Non-Medical Prescriber (V300)
Knowledge & Skills
  • Advanced clinical assessment and diagnostic skills
  • Ability to manage risk and clinical uncertainty
  • Excellent communication and interpersonal skills
  • IT literacy and experience using clinical systems
