Advanced Nurse Practitioner (Part Time)

Harewood Surgery, a highly regarded NHS GP practice serving approximately 3,000 patients across Great Oakley and the surrounding rural communities, is looking for an experienced Advanced Nurse Practitioner to join our multidisciplinary team part time.

Main Duties

• Assess patients presenting with undifferentiated and undiagnosed conditions across all age groups, using comprehensive history taking, clinical examination and clinical reasoning to formulate differential diagnoses
• Initiate evidence based treatment plans, including medication prescribing, lifestyle advice and onward referral where appropriate
• Manage same day acute and urgent presentations, including minor illness, injury, infections, musculoskeletal complaints and exacerbations of chronic conditions
• Conduct routine and follow up consultations for patients with complex or multi morbid long term conditions
• Deliver consultations face to face, by telephone and by video
• Manage clinical emergencies in the practice in accordance with the practice emergency protocol
• Prescribe medicines independently from the British National Formulary (BNF) within scope of competence, including some controlled drugs (Schedules 2 5, subject to regulatory restrictions)
• Conduct medication reviews, optimise medication regimes and deprescribe where clinically appropriate
• Prescribe in accordance with NICE guidelines, local formulary and evidence based practice
• Monitor therapeutic outcomes and adjust prescribing in response to clinical findings
• Maintain prescribing competence through CPD, prescribing audit and peer review

About the Practice

Harewood Surgery, part of Stour Estuary PCN, delivers high quality, patient centred care in a friendly, professional and supportive environment. The team includes 2 GPs, 2 Advanced Nurse Practitioners, a Practice Nurse, a Healthcare Assistant, a Phlebotomist, dispensers, administration staff and allied healthcare professionals.

Job Responsibilities

• Autonomous clinical consultations - assess patients with undifferentiated conditions, formulate differential diagnoses and initiate evidence based treatment plans
• Same day acute care - manage urgent presentations and minor illnesses/injuries, provide medication prescribing and referrals where appropriate
• Chronic disease management - optimise medication regimes for conditions such as diabetes, COPD, asthma, cardiovascular disease and mental health conditions
• Investigations and referrals - order and interpret diagnostic tests, process pathology results, and arrange referrals to secondary and specialist services
• Clinical governance - participate in audits, quality improvement initiatives, develop clinical protocols and report incidents through the clinic's incident reporting system
• Research and evidence based practice - critically appraise research evidence and apply it to clinical practice and service development
• Professional development - maintain CPD, NMC revalidation, and an advanced practice portfolio evidencing practice across four pillars (clinical, leadership, education, research)

Qualifications

• NMC registered Adult Nurse with no restrictions or conditions
• Independent and Supplementary Prescriber qualification (V300)
• MSc in Advanced Clinical Practice or equivalent (or RCN ANP accreditation)
• Training or mentorship qualification (Practice Assessor / Supervisor status) - optional but highly valued

Experience

• Substantial clinical nursing experience post registration, including autonomous assessment, diagnosis and treatment of undifferentiated conditions
• Experience with independent prescribing and medication review across a broad range of conditions
• Experience managing patients with complex, multi morbid long term conditions
• Experience as an ANP in general practice or primary care
• Experience mentoring trainee ANPs or student nurses
• Experience in clinical service development and quality improvement initiatives

Person Specification

• Strong clinical leadership and communication skills - ability to lead, influence and support within a multidisciplinary team and communicate complex information to patients and colleagues
• IT competence - able to use clinical systems for prescribing, record keeping and referrals
• Understanding of clinical governance, audit, SEA and risk management
• Knowledge of the NMC Code, personal accountability at advanced practice level and safeguarding responsibilities for adults and children

Terms and Conditions

• Annual leave: Five weeks plus bank holidays, increasing with service at five and ten years of employment
• Pension: Twelve weeks
• Probationary period: details not specified but required
• Training and CPD: Protected time for CPD, revalidation activities and supervision sessions
• Clinical supervision: Clinical supervision with Prescribing Lead GP; daily access to Duty GP
• Indemnity: NHS clinical work covered under CNS GP
