Main duties of the jobThe post holder will work as an autonomous Advanced Nurse Practitioner within a total triage primary care setting, delivering same day and urgent care for a diverse patient population. The role involves independent clinical decision making, including assessment, diagnosis and prescribing, while working closely with GPs and the wider multidisciplinary team.
Key aspects of the role include contributing to safe and efficient triage, managing acute and complex presentations, maintaining high standards of clinical documentation and governance, and supporting service development within a busy teaching practice. The ANP will also play a valued role in teamworking, clinical discussions and, where appropriate, supporting the education of medical students.

Job responsibilities- Triage and manage undifferentiated, acute, and complex presentations via telephone, online, and face to face consultations.
- Independently diagnose, treat, and prescribe for patients using your prescribing qualification, working within your scope of practice.
- Contribute actively to the MDT, attending clinical meetings and supporting colleagues across the team.
- Manage acute conditions including minor illness, minor injury, respiratory, and musculoskeletal presentations.
- Support the supervision and clinical education of medical students in line with our teaching practice commitment.
- Contribute to clinical governance, significant event analysis, and quality improvement initiatives.
- Maintain accurate, timely records in the practice clinical system (SystmOne).
- Participate in practice meetings and contribute to shaping the future direction of the service.
- Significant post registration clinical experience as a registered nurse, appropriate to advanced practice.
- Demonstrable experience working at Advanced Nurse Practitioner level, managing undifferentiated, acute and complex presentations; experience of working within Primary Care, Urgent Care, Out of Hours or similar patient facing settings.
- Proven ability to work autonomously, including clinical assessment, diagnosis, treatment planning and prescribing.
- Experience of same day or urgent care workload management and clinical prioritisation.
- Experience of working effectively as part of a multidisciplinary team.
- Experience of maintaining accurate clinical records using electronic patient record systems.
- Understanding of and experience working within clinical governance, patient safety and safeguarding frameworks.
- Experience working with a diverse patient population comparable to Southall, with an understanding of the challenges and health needs seen in urban primary care settings.
- Experience working within a total triage or digital first primary care model.
- Experience using SystmOne clinical system.
- Experience managing long term conditions alongside acute presentations.
- Experience supporting or contributing to clinical teaching, supervision or mentoring.Involvement in audit, quality improvement or service development.
- Experience working within a teaching practice or training environment.
- Leadership or informal supervisory experience within a primary care team.
- Registered Nurse with current Nursing and Midwifery Council (NMC) registration and successful revalidation.
- Masters level qualification in Advanced Clinical Practice (MSc).
- Non Medical Prescribing qualification (Independent Prescriber).
- Evidence of significant post registration clinical experience appropriate to advanced practice (normally 3+ years).
- Evidence of ongoing continuous professional development (CPD) and maintenance of a professional portfolio aligned to advanced practice.
- Additional accredited modules or qualifications in:
- Minor illness and/or minor injury.
- Long term condition management (e.g., asthma, diabetes, COPD).
- Teaching, mentorship or clinical supervision qualification.
- Leadership or service development training relevant to advanced practice.
