Main duties of the jobAs an Advanced Nurse Practitioner at Mayfield Medical Centre, you will play a pivotal role in providing high quality, patient centred care to our community.
About usWe play an active role in Paignton and Brixham PCN with one of our Partners sharing the Clinical Director Role. We benefit from a full complement of Allied Health Professional roles, from our passionate Social Prescribing and Mental Health teams through to our Pharmacy team who play a key role in our medicines management. On the day the team consists of GPs and ANPs look after acute presentations whilst maintaining continuity of care.
Job responsibilitiesAs an Advanced Nurse Practitioner at Mayfield Medical Centre, you will play a pivotal role in providing high quality, patient centred care to our community. Your responsibilities will include:
- Work to a defined daily workload with a manageable number of appointments and dedicated time within the clinical session for clinical administration.
- Conducting comprehensive assessments, diagnosing illnesses, and developing treatment plans for patients with acute undifferentiated medical conditions as well as providing chronic care.
- Prescribing medications, ordering diagnostic tests, and interpreting results to facilitate timely and effective patient care.
- Play an active role within our on the day duty team offering safe and effective care to our patients with acute medical needs.
- Take full ownership of admin including pathology and prescriptions, documents arising from your own patient consultations.
- Support other colleagues with any admin duties required, including medicines management.
- Collaborating with GPs, nurses, and other healthcare professionals to coordinate care and ensure continuity of services.
- Educating patients and their families about health promotion, disease prevention, and self management strategies.
- Support in the clinical management of our allocated Care Homes conducting proactive weekly virtual rounds and supporting in virtual ward as part of a dedicated team.
- Conduct a share of the practices urgent home visits to patients as and when required as part of the duty team rota.
- Contribute to the supervision of trainees to enhance learning.
- Participating in quality improvement initiatives and contributing to the development and implementation of evidence based practice guidelines.
- Ensure revalidation and mandatory training remain up to date.

Person Specification Experience- Evidence of appropriate continuing professional development activity to maintain up to date knowledge and competence in all aspects of the ANP role.
- Evidence of working autonomously and as part of a team.
- Proven ability to evaluate the safety and effectiveness of their own clinical practice.
- Experience of acute and chronic illness.
- Experience of working in primary care/GP practice setting.
- An understanding of the QoF framework.
- EMIS experience.
- Full NMC Registration
- Recognised Nurse Practitioner qualification at minimum degree level or equivalent
- Independent prescriber
- Commitment to undertaking further qualification depending on practice need.
