Job Description

An opportunity has arisen for an Adult or Paediatric Advanced Nurse Practitioner to join North Cumbria Primary Care (NCPC), with a position arising in Carlisle Locality.

You will work independently, initiate effective history taking, clinical assessment and diagnosis within your capabilities. You will also implement a range of care interventions including prescribing for patients presenting with undiagnosed problems and/or support patients as part of a care plan.

Main duties of the job

The post holder is an experienced Advanced NursePractitioner who will work as a member of the Practice Clinical Team,assessing patients with undifferentiated and undiagnosed problems, initiatetreatment, investigations, advice and management of problems using professionalautonomous decision making processes, liaising when appropriate with otherhealthcare professionals and service providers to ensure safe and effectivepatient care.

Job responsibilities

Providedappropriate patient care in accordance with practice protocols and policies.

Activelyengage patients and carers in management of their health, promoting health andwellbeing

Workcollaboratively with the multi-disciplinary general practice and primary care teamsto meet the needs of patients, supporting the delivery of policy andprocedures, and providing nurse leadership as required. To work to this levelNMC requirement for advanced practice must be met.

Beresponsible for recognising, negotiating and implementing change processes toenhance the delivery of care, both within the Practice and when completing homevisits.

Contributeto development and delivery of a high-quality innovative service to the patientsof the practice within the community through the implementation of evidencebased clinical practice, supported by continual professional development, andprovide support and mentorship to all healthcare professionals.
