About Us:- Serving approximately 8,300 patients
- Team includes:
- 5 GP Partners
- 1 Advanced Nurse Practitioner
- 1 Practice Nurse
- 2 Chronic Disease Nurses
- Pharmacy team
- First Contact Physiotherapist
- Supported by an experienced administrative team
- Day to day operations overseen by the Practice Manager and Assistant Practice Manager
We are seeking a motivated and experienced Advanced Nurse Practitioner (ANP) to join our dynamic team. As an ANP, you will play a vital role in managing a diverse caseload, working closely with GPs and other healthcare professionals to deliver high quality patient care. The role involves:
- Triaging
- Face to face clinics
- Home visits
- Assess, diagnose, and manage patients with acute conditions
- Perform clinical examinations and arrange investigations/referrals/admissions.
- Be responsible for management of own results and docman
- Ensure appropriate follow up arrangements put in place for patient
- Prescribe as per local guidelines
Systems:We currently use Vision and Docman for clinical and document management.
