This is an exciting opportunity for a motivated, patient centred clinician to play a key role in delivering exceptional primary care to our growing community.
As an ANP at Priory you'll lead on the management of Long Term Conditions, with a particular focus on Diabetes, while also contributing to our same day acute triage service. You'll use your advanced clinical skills to assess, diagnose, and treat a wide range of conditions, working autonomously but with the full support of our experienced multidisciplinary team.

Main duties of the job As an Advanced Nurse Practitioner at Priory Medical Centre, you'll play a pivotal role in delivering high quality, patient focused care across both planned and same day services. Your day to day work will be varied, autonomous, and rewarding.
In June 2021, the practice moved into a brand new, purpose built surgery designed with patient care in mind. The new building will be a modern, energy efficient & fit for purpose building that will enable the practice to provide better services to our patients. As a result of having the new facilities available, the practice closed the main Cape Road surgery, along with the Cape Road branch surgery. The new facilities will act as the main surgery, with a branch surgery still available to patients on Woodloes Park at the following address: 1 Brese Avenue, Woodloes Park, Warwick, CV34 5TS.

Main duties of the job As an Advanced Nurse Practitioner at Priory Medical Centre, you'll play a pivotal role in delivering high quality, patient focused care across both planned and same day services. Your day to day work will be varied, autonomous, and rewarding.
- Take the lead in managing Long Term Conditions, with a specialist focus on Diabetes, providing comprehensive reviews, personalised care plans, medication optimisation, and proactive health coaching. Your expertise will help patients take control of their health and improve outcomes across our population.
- Run same day acute triage clinics, assessing, diagnosing, and treating a broad spectrum of undifferentiated conditions. You'll use your advanced clinical skills to make safe, effective decisions, request and interpret investigations, prescribe independently, and refer or escalatate when appropriate.
- Work closely with our GPs, nurses, pharmacists, paramedics, and wider multidisciplinary team, contributing to a supportive, progressive environment. You'll also be involved in continuity of care, safeguarding, quality improvement, and helping shape service delivery as primary care evolves.

Job responsibilities The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Develop, implement and embed health promotion and wellbeing programmes
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to their health
- Provide advanced, specialist nursing care to patients as required in accordance with clinical evidence, NICE and the NSF
- Support wound care (ulcer/Doppler etc.) by providing advice to the nursing team
- Provide specialist clinics such as contraception initiation etc.
- Request pathology services as necessary
- Process and interpret pathology and other test results as required
- Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
- Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively
- Maintain chronic disease registers
- Develop, implement and embed an effective call/recall system
- Be responsible for the implementation of an effective immunisation programme
- Chaperone patients where necessary
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognise, assess and refer patients presenting with mental health needs
- Support vaccination programmes for adults and children
- Be an Independent Nurse Prescriber, adhering to extant guidance
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
- Contribute to practice targets (QOF etc.), complying with local and regional guidance
- Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
- Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
- Support the clinical team with all safeguarding matters in accordance with local and national policies
- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
- Deliver opportunistic health promotion where appropriate
- Act as the audit lead, effectively utilising the audit cycle
- Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary
- Monitor and ensure the safe storage, rotation and disposal of medicaments
- Undertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staff
- Participate in local initiatives to enhance service delivery and patient care
- Support and participate in shared learning within the organisation
- Develop an area of specialist interest, taking the lead within the organisation
- Continually review clinical practices, responding to national policies and initiatives where appropriate
- Participate in the review of significant and near miss events applying a structured approach i.e., root cause analysis (RCA)
- Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required
- Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation
- Registered Nurse with Nursing and Midwifery Council
- Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
- Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
- Qualified Independent Nurse Prescriber on the NMC register
- Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
- Meets the standards for registered ANP working at advanced level
- Teaching qualification

- Wound care/removal of sutures and staples

- Broad knowledge of clinical governance
- Ability to record accurate clinical notes
