Advanced Nurse Practitioner

We are looking for an experienced ANP to join our friendly, forward-thinking Practice. The successful candidate will complement our existing ANP team, and the wider clinical team in providing high quality services to our population.
This new post presents an opportunity to establish an acute illness home visiting service in response to daily visit requests from patients who are unable or are too unwell to come into the Practice. You will also provide face to face clinics at the Practice and cover other ANP colleagues during periods of absence.
Main duties of the job- Assess, diagnose, and implement treatment/interventions and care for patients presenting with acute and chronic illnesses; and complex need, ideally covering all ages from children to the elderly.
- Visit, assess and treat complex frail patients with multiple co-morbidity who are housebound, in residential care and care homes, and designated visits from daily requests as deemed appropriate following triage.
- Maintain accurate, up to date patient records, documenting medical history, treatment plans and folllow up care.
- Working with ANP colleagues in maintaining a frailty service and minor illness clinics, and ideally the triage of home visits and daily requests for urgent appointments.
We are a large Practice of 15,000 patients in SunderlandNorth. The Practice is part of the Deep End Network delivering care to the mostdeprived in the area.
Our team consists of 3 GP Partners, 5 Salaried GPs, 5 ANPs,2 PNs, 1 NA & 1 TNA, 1 HCA, 1 Phlebotomist, 1 Clinical Pharmacist, 1Pharmacy Technician, a large Admin Team and the PCN Teams.
Job responsibilitiesAssess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with acute illnesses for children through to the elderly.
Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly.
Assess,diagnosis, plan, implement and evaluate interventions/treatments for patientswith complex needs.
Providehome visits to patients of all ages as deemed appropriate following triage.
Liaisewith other providers to ensure continuity of care to this group of patients.
Proactivelyidentify, diagnose and manage treatment plans for patients at risk ofdeveloping a long-term condition (as appropriate).
Diagnoseand manage both acute and chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan.
Prescribeand review medication for therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice.
Workwith patients in order to support compliance with and adherence to prescribedtreatments.
Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions.
Supportpatients to adopt health lifestyles and apply principles of self-care.
Person Specification Other attributes- Effective time management and organisational skills
- Demonstrate personal accountability, emotional resilience and ability to work under pressure
- Current clean driving license and access to own transport for business use
- Evidence of revalidation
- Registered general nurse with Nursing and Midwifery Council (NMC)
- Masters degree in advanced practice
- Independent prescriber
- Qualified triage nurse
- Teaching qualification
- Understanding of general practice and the wider NHS
- Experience of practice within the four pillars
- Experience of independent prescribing and undertaking medication reviews
- Experience in using primary care clinical systems, EMIS preferable.
- Experience of working as a practice nurse or community nurse
