Job Description
An exciting opportunity has arisen for a dynamic, enthusiastic, motivated, highly experienced nurse with palliative and end of life care practice and knowledge. The post holder will join our well-established expert community and ward nursing teams and medical services as a qualified Advanced Nurse Practitioner.

This post will provide varied senior experience in a multidisciplinary working environment both within the community services and the 18-bed ward, which will be undergoing redevelopment September 2025 - March 2026

We are looking for a candidate who has advanced communication and clinical assessment / prioritisation skills, as well as an understanding of the complex needs of those receiving expert palliative care /end of life care

In addition, competency in evidence-based practice and the ability share a sound knowledge of current initiatives and research developments in palliative care.

Extensive autonomous communityexperience would be additionally valuable to this post and successful non-medicalprescribing qualification essential to support senior level decision making.

Main duties of the job As an expert, will deliver high quality care to individuals,enable other practitioners to maintain professional expertise whilst exercisinga high degree of autonomy. This will include 50% direct clinical practice andinclude non-medical prescribing.

Education, training and development function. Providespecialist training and education, establishing formal links with localeducation providers. They will participate in the development of studentsworking towards professional registration as well as the learning and developmentof qualified staff. Professional leadership and consultancy function. Demonstrateleadership skills to support and motivate others and to continuously improvequality of care and standards of practice, providing a source expertise andknowledge.

Practice development, service evaluation andimprovement function. Nurse practitioner will contribute to development ofprofessional standards, through the promotion of evidence-based practice. Theywill also conduct research and evaluation of practice within their specialistfield.

Research and Quality function. Collaborate and workalongside colleagues to identify and support Quality and Research activity andinitiatives.

MAIN DUTIES AND RESPONSIBILITIES:

This is a key post within the specialist area and willhave a role in defining, shaping and developing all services, including thosein community settings.

Clinical Practice

TheANP job plan will include designated sessions for delivery of direct, expertclinical care and evidenced-based, complex decision-making

The ANP will promote clinical pathways acrossmedical and nursing teams, practicing autonomously, in accordance with scope ofpractice and professional accountability, at an advanced level

Rolemodel advanced nursing practice for all nursing staff and the multidisciplinaryteam

The post holder will provide systemleadership, supporting Clinical Specialists to realise their full potential asautonomous practitioners to ensure seamless delivery of care and high-quality outcomes

The post holder will act as an innovator,change agent and role model to support and influence clinical pathway redesignwithin services including quality, implementation of national initiatives,service reconfiguration and new digitally enabled models of care

The post holder will work closely within anMDT in addition to key relationships above

Expertadvice and support to the clinical and medical teams

Integrateholistic assessment, diagnosis, plan of care, treatment and discharge inaccordance with the evidence base

Participatein quality initiatives to achieve evidenced based clinical standards

Recogniselimitations in scope of practice and address theoretical and practice gapsthrough continuing education and consultation with medical colleagues.

Clinicalleadership of the development of nurse-led practice, including strengtheningnursing beyond the current scope of practice and generating innovation andtransformation in service models and delivery

Useadvanced communication skills in all challenging contexts and communicate information that may be sensitive andcomplex

Recordkeeping and data handling in accordance with hospice policy

Promotean internal and external understanding of the role to increase the awareness ofthe scope of practice

Lead on the production of an annual report, demonstratinggoals and outcomes aligned to the organisational strategy

Work in partnershipwith the quality lead to implement a quality management system and utilisefindings to initiate change in practice

Initiate and fosterlinks with higher education institutions and other organisations in relevantto research

Contribute to serviceplanning and budgetary processes through use of data and specialistknowledge

Develop datacollecting systems that will inform and evaluate the service provided

The post holder will be akey member of the Research Interest and Audit Groups, contributingexpertise and mentorship in these areas.

Education

The post holder will have an educationalcommitment to ensure clinical pathways and practices are enveloped in all stafftraining programmes

Collaborateto facilitate the education and professional development of all nursing gradesand other healthcare professionals.

Participate in facilitating learning and education (bothundergraduate and post-graduate levels) and other health professionals in theclinical area.

Contributeto the professional body of knowledge and clinical practice, nationally andinternationally, by writing, publishing and presenting outcomes

Actively promote the development of alearning culture, to drive high performance, improvements and innovation

Person Specification Qualifications
  • Registered Nurse with evidence of PIN and revalidation.
  • Significant demonstrable experience of practice in palliative care at Band 7 or above.
  • Masters degree in relevant subject/significantly working towards.
  • Accredited management training course or significantly working towards.
  • Teaching qualification: Post Graduate Certificate in Education or equivalent.
Experience
  • Ability to coordinate the completion of several projects simultaneously.
  • Ability to prioritise and meet deadlines.
  • Ability to compile, analyse and disseminate complex information.
  • Registered nurses working at this advanced level -
  • are accountable practitioners, working within the boundaries of their profession.
