Advanced Nurse Practitioner - Walk-In Centre

We are currently seeking an experienced and highly skilled Advanced Nurse Practitioner to join us to work in a Walk-In Centre in Leeds (two sites available). As an Advanced Nurse Practitioner, you will play a crucial role in delivering high-quality primary care services to patients in a fast-paced and dynamic environment.

Responsibilities:

• Provide advanced clinical assessments and diagnosis for patients attending the Walk-In Centre
• Independently manage and treat patients with acute and chronic illnesses within your scope of practice
• Prescribe medication and refer patients for further investigations or specialist consultations as required
• Collaborate with the wider healthcare team to ensure seamless patient care and effective communication
• Demonstrate excellent communication skills and empathetic approach in dealing with patients and their families
• Maintain accurate and up-to-date patient records using electronic medical records systems

Requirements:

• Valid NMC/HCPC Registration
• Hold Level 7 Minor Injury and Minor Illness
• Must be a Prescriber
• See and treat all patient groups
• Proven experience working as an Advanced Nurse Practitioner in a primary care or Walk-In Centre setting
• SystmOne experience, with a valid Smart Card
