Advanced Nurse Practitioner - Hospital at Home

The shift pattern for this post is 5 days over 7, including out of hours periods.
An exciting opportunity has arisen within Glasgow City HSCP for an Advanced Nurse Practitioner (ANP) to support the Glasgow City Hospital at Home service. This service provides acute level care within patients' own homes, focusing on respiratory illnesses and other acute or exacerbated medical conditions. By delivering early assessment, diagnostics, and evidence based interventions in the community, the service aims to reduce unscheduled hospital admissions, improve patient experience and support timely discharge, thereby alleviating pressure on inpatient bed capacity and reducing emergency department waiting times across Glasgow City.
ResponsibilitiesThe post will provide clinical, operational and professional leadership. You will be a registered nurse educated to masters level in advanced practice, enabling competence in clinical assessment, clinical reasoning, judgement, diagnostic decision making, anatomy and pathology, non medical prescribing (V300), and leading delivery and evaluation of care. You will have significant experience as an ANP in either acute or community settings. The role will integrate into the development of the Hospital at Home Service and will involve working across disciplines and teams, with a proven track record of service development and team leadership.
Qualifications• Registered nurse with a masters level education in advanced practice. • Competence in clinical assessment, clinical reasoning, judgement, diagnostic decision making, anatomy and pathophysiology. • Qualified for non medical prescribing (V300). • Significant experience as an ANP in acute or community settings.
