Advanced Nurse Practitioner - Argyle Medical Group

We are looking for a practitioner who will provide patient-centred clinical care in care homes and other relevant housebound patients. This role focuses on assessment, treatment, monitoring and care-coordination for patients with acute, chronic or complex health needs. The practitioner will work as part of the primary care multi-disciplinary team.
Main duties of the jobCommunity based role primarily visiting patients in a care home setting
Telephone consultations with care homes where appropriate

About usWe are a large GP Practice of 22600 patients in rural west wales which has adapted with a multidisciplinary team of GPs, Pharmacists, ANPs, Chronic Disease PNs, PNs, HCA/Phlebotomists. We have two GP trainees within the Practice and support GP Registrars. Argyle has a higher than average disease prevalence for chronic disease with social deprivation, with mental health and drug dependence all being definite factors our specially run Chronic Disease clinics are highly effective in the treatment of these patients. As a member of the South Pembrokeshire Cluster we continue to use the skills of our physiotherapist. We also use the facility of our youth Worker and ICT team referring vulnerable/housebound patients through the MDT. We aim to work closely with our local community and are a Practice holding a Silver award for our Carers we have also liaised closely with our local schools delivering educational sessions to the children. Whilst we are undoubtedly a very busy Practice because of our patient list size we have a very supportive team and a great social committee.
Job responsibilitiesKey responsibilities
Clinical care
Care co-coordination
Collaborate closely with GPs, nurse practitioners, nurses, social workers and allied health workers
Communicate assessment findings and care plans clearly to the primary care team
Support care transitions following hospital discharge or emergency visits
Facilitate referrals to community services and resources as needed
Documentation and Quality
Accurately document assessments, interventions and outcomes in the electronic medical record
Participate in quality improvement initiative and projects
Follow clinical protocols, infection control and patient safety guidelines
Build trusting relationships with patients, carers and families in the care home setting
Provide culturally sensitive, trauma informed and patient centred care
Support advance care planning and goals of care discussions as appropriate
Professional practice
Practice evidence-based medicine and adhere to clinical guidance
Maintain current licensure, certification and continuing education requirements
Participate in team meetings, case conferences and ongoing training
Person Specification Qualifications- Registered paramedic with appropriate qualifications
- Experience in primary care, geriatrics, community-based care
- Experience working with a multidisciplinary team
- Knowledge of EMIS
- Comfortable working independently in community setting
- Strong problem-solving and adaptability in non-traditional care environments
- Compassionate, professional and patient focus approach
- Strong team player with a commitment to multidisciplinary working
- High level of organisation and documentation skills
- Motivated to develop role in primary care and work with care homes to provide excellent patient care
